This employment agreement for research has been made this Day ............., date .......................,

below, .................................. .............. Head of Research and Community Service Intitusion at
STIKes Wijaya Husada Health Science Intitute, as the FIRST PARTY. And Researcher /
Servant, ............................... here in after referred to as SECOND PARTY. In this case acting on
behalf of THE FIRST PARTY and THE SECOND PARTY agree to enter into a work contract
for the Research / Community Service activities.

Article 1
The Second Party will carry out research / community service activities under the title

Section 2
The activities mentioned in Article 1 will be carried out by the SECOND PARTY starting on the

Article 3
The FIRST PARTY provided funds of Rp. ..............., - (................................ ..................) to the
SECOND PARTY to carry out these activities in Article 1. The intended source of costs
originates from the Budget Post of the Institute of Research and Community Service Institusion
at STIKes Wijaya Husada Bogor and other sources which is not binding based on the SECOND
PARTY's efforts.

Article 4
Payment of the funds in Article 3 will be made in two terms as follows:
1. Phase I 70% of the total research funding needed is Rp. ......................., -
(........................ ....................................) after the SECOND PARTY submitted the proposal for
the activity in Article 1 which has been granted approval from the head of his work unit or
responsible (Head of Study Program / Dean);
2. Phase II 30% of the remaining total approved funds amounting to Rp ...................., -
(............. ...............................................) after BOTH PARTY submit final reports of research /
service activities and evidence has been published in the journal.

Article 5
(1) The SECOND PARTY shall carry out the activity in Article 1 within the time specified in
Article 2;
(2) The FIRST PARTY will evaluate the implementation of research activities as mentioned in
Article 1;
(3) The FIRST PARTY will postpone payment of the SECOND PARTY for late submission of
activity report;
(4) The amount of research costs can be seen in the Proposal.
