Vasa Nervorum as the Source of Corkscrew Collaterals in Thromboangiitis


Obliterans (Buerger’s Disease)

A 57-year-old male heavy smoker with known Buerger’s disease presented with progressively worsening ulcers on the tips of his
toes. Foot pulses on both sides were not palpable. Pathognomonic corkscrew collaterals were noted on computed tomography
angiography. However, those collateral vessels were identified inside (arrow) and around (arrowhead) neural structures (sciatic
nerve) near the occluded arterial segments (dashed arrow; curved arrow indicates popliteal vein) on both sides.This finding suggests
that the corkscrew collateral vessels in thromboangiitis obliterans may originate from vasa nervorum rather than vasa vasorum.
