1) Be prepared to learn
2) Be kind and courteous to others in the classroom
3) Raise your hand and wait to be called on to speak
4) Respect your supplies and the supplies of those
around you
5) Ask before leaving your seat or desk
 Rewards Individual
•Dojo points awarded
•Once a week, Dojo points can be redeemed for prize box, lunch with the
teacher, free time activity of choice(must have enough points earned for
selected choice)
• Positive note home
Whole-Class
•Class points awarded
•Extra recess when enough points earned
•Extra dance break at the end of the week based on class points earned
during the week
 Consequences
Individual- Whole-Class-
1. First step: Warning 1. First step: Warning
2. Second step: Lose Dojo point 2. Second step: Lose Dojo class
3. Third step: Behavior form point
4. Fourth step: Call/email home 3. Third step: Email home
5. Fifth step: Meeting with 4. Fourth step: No extra dance
parents/principal break at the end of the week

Answer all questions in complete sentences when told to by the teacher.

1) What was I doing that caused me to get a behavior form consequence?

2) What choice can I make that would be better?
3) What can I do to correct my choice next time?
