3. Can you describe a personal accomplishment, event, or realization that you are
most proud of?
4. What characteristics and values do you possess that will help you to become
successful in your studies?
5. Have you been a leader? How would you describe your leadership style?

FORMAT GUIDE
1. Allot one (1) inch margin for all sides (bottom, top and sides).
2. Use Arial or Times Roman Font 12.
3. Use double space.
4. At the top left side of the first page, write your name on the first line followed by
your 10-digit Application Number.
5. Put a page number at the bottom right of your paper.
