CASE STUDY 3

A 69-year-old man with no prior medical history presents over the course of several months with
new-onset nonspecific joint pain and swelling without evidence of erosions. The joints involved
include his wrists, elbows, hands, and knees.Because of the severe pain, the patient was
placed on steroids and was given analgesics, some of which contained acetaminophen. The
patient also took additional analgesics to help control the pain. Seven months ago, he began
noticing stiffness in both hands in the morning that lasted longer and longer. Stiffness now
lasted more than 1 hour every morning and included hands, wrists and ankles. He also had
increasing difficulty standing for long periods at work or at home due to foot and ankle pain. He
began taking ibuprofen 800mg 3 times daily and found it helped him get through his day with
less pain and stiffness. Three months ago, he noticed pain in his right and left shoulders during
daily chores. He also began feeling extremely tired and short tempered. He had no energy to do
his usual activities. Ibuprofen was no longer very effective for his pain or stiffness. The patient
was admitted to the emergency room for weakness and hematemesis. On admission the
patient's hematocrit was 21%. Endoscopy demonstrated a large bleeding ulcer and fresh blood
in the stomach and proximal duodenum. A 2-dimensional echocardiogram before starting
immunomodulators was performed, showing a poorly visualized pericardial structure in the
subcostal view that appeared to be compressing the right ventricle.
