Safety Alert – A Hand Injury Accident

Accident Brief Description:

1. The accident took place on September 4 th, 2021 a drilling rig was towed, and a pile of fixing
blocks need to be taken out before the rig is in place, in order to lower the pile leg of drilling
rig.

15:00: Driller name Huang was taking out the fixed pile block, his left hand was holding on
the handle of the pile fixing block, and his right hand using the hammer to knock the side
of the pile fixing block. Due to the deviation of the hammer hitting, the end of his left little
finger was injured.
Preliminary Cause Analysis:
- Operators may make an error
- Operators may not concentrate during operation
- Improper working methods ma be adopted when taking out the fixing pile blocks on site
- The basic operation skills of operators may not be proficient enough

Safety Alert:
- Worker should identify any hazard potential and risk, therefore understand the right method
step by step how to do that thing
- Strengthen employees training on hand injury prevention, and improve employees safety
awareness and risk identification
- Enhance the intrinsic safety of on-site equipment, tools and places effectively protect the
parts held and operated by workers
- Strengthen basic skill training of on-site operator to ensure that each post master the
common skills and avoid accidents
- All units shall solidify the safe working methods, experience and risk control measures, update
them into the work instruction
