Dialogue Patient And Nurse

Nurse : (tok... tok... tok.....) Assalamualaikum...., Good Morning, Mrs.

Sonia. My name is Neli Riyani Putri, I am nurse on duty today from 06.00
– 12.00 am and I’ll be looking after you for this morning.
Patient : Waalaikumsalam......, Good Morning Mrs Neli
Nurse : Would you like to tell me where do you live?
Patient : Yes, I live in the city of Padang which has the address at Limau
Manis
Nurse : Owhh Limau Manis, oke. Are you feeling better now?
Patient : No, Mrs. Neli I have a headache. This feels so dizzy
Nurse : Has Sonia’s mother taken medicine?
Patient : No, I just finished eating sus
Nurse : Okay, I will help Sonia’s mother to take the medicine, after taking
the medicine don’t forget to take a break, ma’am

Dialog 1 :
a conversation between a nurse and a patient in a room in a hospital.
In that situation the nurse is seen greeting the patient "good morning" Mr.
burton. then the nurse introduced herself she was called nurse Hannah who
was going to treat mr burton this morning. Mr Burton greeted nurse
Hannah back with "good morning." After that nurse Hannah explained the
first thing I will do is check your details, that's okay. and the patient agreed
to do the examination, then the nurse asked the patient to tell the patient's
full name, the patient answered his name was William Burton, the nurse
also asked the patient's date of birth, the patient was born on December 15,
1988, then the nurse asked the address of the patient's residence, the patient
replied that he lived in its 17 sunny way Newtown, but the nurse said it was
a different address, then the nurse asked for the patient's current address,
the patient replied that he had moved 3 weeks, the nurse would change the
patient's new address, then the nurse asked again if Mr Boton's cellphone
number was still the same , said mr burton yes it's still the same. the nurse
gave one last question before she made the admission observation to mr
borton. asked if mr borton had any closest relatives, mr borton said he has a
girlfriend then he will give his cellphone number now, then the nurse
thanked mr burton.

Dialog 2 :

a patient who comes to see a doctor for tests and consultations about
his illness the client wants to do an x-ray test and asks what the disease is
because the client also doesn't know the disease he is suffering from then
the doctor asks whether the patient has a history of the disease or not and
the client turns out to have had pneumonia which he suffered from 2 years
ago and had appendix surgery 14 years ago and the client also has a peanut
allergy
