Scenario SET A:

A 59-year-old obese male patient was admitted at the hospital with left sided body weakness, nape
pain, and slurring of speech. Upon assessment, the baseline vital signs include a Blood pressure of
220/120mmhg, pulse rate of 84, and respiratory rate of 20.

After the initial assessment, the physician orders for series of laboratory tests. The laboratory
results included a normal CBC and total cholesterol of 240.

The physician orders for the following medication:


1. Enalapril 40mg/tab P.O OD
2. Atorvastatin 20mg/tab P.O OD at bedtime

Enalapril
Drug Classification: (1 point)

Angiotensin-converting enzyme (ACE) inhibitors.

Drug Action/s:

Enalapril is used alone or together with other medicines to treat high blood pressure
(hypertension). It works by blocking a substance in the body that causes the blood vessels to
tighten. As a result, enalapril relaxes the blood vessels. This lowers blood pressure and increases
the supply of blood and oxygen to the heart. Enalapril is also used in combination with other
medicines to treat congestive heart failure and cut down on the number of hospital visits for
heart problems.
Dose, frequency and route of administration (doctor’s order): (1 point)

Enalapril 40mg/tab P.O OD

Indication/s (reason/s) for administering the medication to the patient: (1 point)

 It is administered to the patient because Enalapril controls hypertension and provides blood
pressure lowering effects. It is a treatment for hypertension alone or in combination with
other antihypertensives, especially thiazide-type diuretics. It also treats heart failure and
treat asymptomatic left ventricular dysfunction.

List two (2) important nursing responsibilities you have to consider when administering the

 Monitor patients on diuretic therapy for excessive hypotension after the first few doses
of enalapril.
 Monitor patient closely in any situation that may lead to a drop in BP secondary to
reduced fluid volume (excessive perspiration and dehydration, vomiting, diarrhea)
because excessive hypotension may occur.

List three (3) patient health teachings in relation to the medication: (3 points)

 Do not stop taking the medication without consulting your health care provider.
 Inform patient that light-headedness and fainting may occur, especially during first few
days of therapy. Advise him to change position slowly and avoid hazardous activities
until drug’s CNS effects are known.
 Advise patient to consult prescriber before using salt substitutes, potassium supplements,
or other drugs (including OTC drugs) while taking drug.
