PARENTAL CONSENT AND WAIVER

I/We, the parents/legal guardian of the student being registered, hereby give our consent for our child
to participate in this competition/training and other related activities of ASIAN MATHSCI LEAGUE INC.
(AMSLI), (collectively referred to as the “Activities”).

We acknowledge and accept that our child’s participation in the Activities is entirely voluntary. We
understand that participation in the Activities may involve risk of physical injury, illness or property loss, and
despite safety precautions, AMSLI cannot guarantee at all times the safety of our child. We voluntarily assume
these risks when we allow our child to participate in the Activities.

We understand that rules and regulations shall be in effect to ensure the safety of everyone in the
venue where the Activities are held. We undertake that our child shall strictly comply with these rules and
regulations.

We also understand that AMSLI and its third-party partners may collect, use, process, disclose, publicize
any information collected and produced before and during the participation in, and administration of, said
Activities as they may deem necessary, and that in pursuing this registration, we agree to these, in accordance
with the Data Privacy Act of the Philippines at the date of registration. This consent extends to the use of
information as may be disclosed to AMSLI-affiliated Competition Organizers, Training Providers, Database
Managers and Coordinators, or disclosed to public in so far as notification and publication of results,
promotion, or media purposes of the Activities are concerned. These data may be kept for at least 12 years.

AMSLI does not provide health and accident insurance for trainees attending the Activities, and we
understand that any medical expenses, property loss, or other personal expenditures that result during or
from the Activities, are to be borne by the trainees, or by their parent or guardian. We also hereby consent,
give authorization to, and release from liability, AMSLI or any of its representatives to secure any emergency
medical treatment in event we are unable to do so, and we agree to be responsible for the costs thereof.

In consideration of the opportunity afforded by AMSLI in training our child, and with full knowledge
and acceptance of the risks associated with the Activities, we hereby release, indemnify and hold AMSLI, its
trainers or mentors, staff, trustees, officers, volunteers, and agents free and harmless from any and all actions,
damages, claims, suits and costs of any nature arising from our child’s attendance and participation in the
Activities.
