AFFIDAVIT OF EXPLANATION

I, XXX, of legal age, Filipino citizen, married, and a resident of ______________, after
being duly sworn to on oath, hereby depose and say that:

1. On July 13, 2017, I went to the Department of Foreign Affairs (DFA) in Manila,
Philippines, to process my passport application because I was planning to work
abroad, specifically in Qatar;

2. However, I never went back to DFA Manila after July 13, 2017 because the agency
that was processing my employment abroad suddenly closed down;

3. On September 1, 2021, I went to DFA in San Fernando City, La Union, to process

another passport application, this time for possible employment in Saudi Arabia.
However, the DFA employee therein notified me that I already have an existing
passport, issued under my maiden name XXX, with Passport No. 123456;

4. I was married on March 12, 2017, at _________, to YYY. This was four (4) months
before I processed my passport application at DFA Manila. However, I had to use
my maiden name despite being already married because I still had not updated
my existing identification cards and other records at the time to XXX;

5. I did not know that DFA Manila had issued a passport with Passport No. 123456
because I immediately went back to Santol, La Union, after the employment
agency closed down. This is also why I was never able to claim said passport
from them;

6. That I am executing Affidavit to attest to the truthfulness of the foregoing and for all
legal intents and purposes.

IN WITNESS WHEREOF, I have hereunto set my hand this 27th day of September
