1. Respectfully requests for Drug Test on the living person of PAMPY GUILLER
PAMPLONA, 24 years old, single, born on January 12, 1995 in Upper Bonifacio St., Baguio
City, jobless, and residing at No. 200 Marcos Highway, Baguio City, who was arrested
during a buy-bust operation conducted against him on February 15, 2019 at about 3:15 p.m.
at 200 Marcos Highway, Baguio City.

2. The purpose of the examination is to determine whether or not the arrested suspect is a user
of dangerous drugs known as Methamphetamine Hydrochloride or “Shabu” or any other
Illegal/Prohibited drugs.

3. Further requests that this Office be furnished of the results of said examination for our
investigative reference and any other legal purpose it may serve.
