SUBJECT : Final Investigation Report on a Found Cadaver

II. MATTERS INVESTIGATED:


4. To determine facts and circumstances of the incident.
5.To determine other relevant matters that would require further police actions.
6. To determine the identity of the victim.

III. FACTS OF THE CASE:


7. On 7:20 am, 23 August 2019, MR. FERNANDO ARELLANO Y DULCE and MR.
LEO BAKANIS saw a cadaver located along a grassy portion at Outlook Drive, Lualhati
Barangay, Baguio City. while the two witnesses are walking and to get pee on a grassy portion.
They saw the body of a person wearing a green T-shirt, gray slacks and a black rubber shoes
which lay in a supine position on a grassy area. The two witnesses went near to the body and
they noticed that the body had no signs of life.

8. The SOCO TEAM led by PCL JULIE CENTENO proceed to the crime scene and
they process the crime scene and they found a Malathion Pesticide bottle and a written farewell
note written in a paper

9.Initial finding of PSUPT. RODRIGO LEAL, the victim was dead for 8-12 hours and
the cause of his death was multiple organ failure secondary to pesticide toxicity.

10. The further investigation confirmed that the found cadaver was identified
as Mr.Juanito Baltazar Y Quino, 34 years old, married, native of Novaliches Quezon
City, a salesman at Sunrise Lumber and Construction, and a resident of Purok 11, Irisan
Dumpsite, Baguio City, currently living with his wife Mrs. Catrina Baltazar Y jacinto, 28
years old, and a native of Laoag, Iloco Norte, and a worker of Baguio’s Angel, Baguio
City.

IV. DISCUSSION
11. According to the initial investigation, Mr. Juanito Baltazar Y Quino found
out that his wife Mrs. Catrina Baltazar Y Jacinto was having an illicit affair with his own
best friend, Ricardo Carino, 30 years old, a native of La Union, single, and a salesman
at Sunrise Lumber and Construction.

12. A farewell note written in a piece of paper addressed to his wife


expressing “mas mabuti nlng na mamatay ako, para hindi ko nakikita ang pagtataksil
mo. Masakit tanggapin ang ginawa mo dahil minahal kita ng lubusan. Bakit mo nagawa
eto, binigay ji lahat sau, ang aking puso’t kaluluwa..”

V. CONCLUSION:
13. The identified cadaver as Mr. Juanito Baltazar Y Quino was able to be identified
by his wife upon arrival at the funeral.

14. Initial findings of P/SUPT Rodrigo Leal presumed the victim dead for about 8 - 12
hours. A farewell note was found at the crime scene expressing sympathetic farewell to
commit suicide.

VI. RECOMMENDATION:
15. Wherefore, in view of the foregoing, the undersigned recommend this case is
closed.
