SUBJECT : Request for Cross Matching Examination

1. Request to conduct cross matching examination on the accompanying specimen to determine

whether the fingerprints describe below is owned by the suspect or otherwise.
a. NATURE OF THE CASE: Alleged Homicide Incident
b. VICTIM : Masie Phag (Deceased)
c. SUSPECT : Elmer Tado
d. TDPO : On or about 5 PM of June 15, 2019 at
Lendi Hotel, Legarda Road, Baguio City
e. SPECIMEN : a) One “1” fingerprint specimen lifted from the handle
of the knife taken from the crime scene, placed in a small envelope marked “NRA 10/15/19”.
b) Fingerprint specimen taken from the suspect
marked “S1”

2. Further request that this office be furnished a copy of cross matching examination result for our
reference.
