FACTS OF THE CASE:

(SHOOTING INCIDENT): On or about 11:00 PM of November 15 2019 a cellphone call
received by this Police Station from Barangay Kagawad María M. Clara and reported that a
Robbery incident transpired at the Gensan Ville phase 1Barangay Bula General Santos City.
Upon receipt of the report, the team led by PSSg Khali S. Bangon proceeded at the said place
and upon arrival thereat, said team together with the said Barangay Kagawad the Alleged
Suspect come out of the house of the suspect while carrying A bag the suspect identified as
Capitán P. Tiago 24 years old, married, and also residence of the said Place.

Entry Nr. -0555 / 15 Nov. 2019 /12:00AM: Person of María M. Clara Call on this office and
reported that the suspects who Robbed the house is as Capitán P. Tiago

-0320/15 Nov. 2019/1:00PM: : Initial investigation disclosed/conducted that as Capitán P. Tiago

went to the house of Crisóstomo S. Ibarra while he was fall asleep. The Said suspect grab and
get the opportunity to get inside the house of Crisóstomo S. Ibarra without no more doubt. And began
to Robbed. But the he seen by CCTV Camera

1. Reference Blotter Entry No. 0555 dated November 15 2019.

2. Please be informed that on or about 11:00 PM of this date that a shooting

incident transpired at the Gensan Ville phase 1Barangay Bula General Santos City of where in
the victim Crisóstomo S. Ibarra ,50 years old, married, Engineer and a resident of Gensan Ville
Phase 1 Barangay Bule General Santos City, while the at suspect was identified Capitán P.
Tiago 24 years old, married, and also residence of the said Place.

3. Investigation conducted disclosed that on the said date, Capitán P. Tiago went to
the house of Crisostomo While he is fall asleep. The suspect was arrested and recovered from
his direct possession and control is one (1) 9mm beretta pistol with serial number ABC 12345
and one (1) magazine loaded with 12 live ammos and immediately brought to the police station
for inquest proceeding.

Dona Soledad Phase 3B Barangay Labangal General Santos City , after having been duly
sworn in accordance with law, hereby depose and state:

1. That I am NOT the same person as one Binugha who was charged under I.S. No.063
/ Criminal Case No.0023 before for the crime of MURDER;

2. That my middle name is Sugnud and not Binugha .

3. That I have never been the subject of any criminal complaint, charge or proceeding
before any prosecutor or court;

4. That I am executing this Affidavit to certify or attest to the truth of the foregoing facts
and for purpose of denying that I am the who is the subject of the aforementioned case
in the I.S. No. 063 / Criminal Case No. 0023.

 We PStaffSrgt Sabwan S Bukog, PtrlMan Prayd S. Cheken and PtrlMan Lungag U.
Kan-on , legal ages and presently assigned at Makar Police Station Isabela after having been
duly sworn to in accordance with the law, do hereby deposed and state the following:

1. That on or about 6:00 PM of September 05, 2019 a cell phone call received by
personnel of Makar Police Station from Brgy. Kagawad ADOBO B. CALDERITA of Dona
Soledad Brgy. Labangal G.S.C. that PoliceM/Sergeant Caldero Sugnud Y Ininit, 31 years old,
married, presently assigned at Makar Police Station and a resident of Dona Soledad Brgy.
Labangal G.S.C. shot Tinae S. Tungol in his head that caused his instant death with the used
of calibre 9mm at the barangay hall of the said place. Upon receipt of the report, team led by the
COP proceeded at the said place and upon arrival thereat, said team together with the said
Brgy. Kagawad saw PoliceM/Sergeant Caldero Sugnud Y Ininit in the Barangay hall and
arrested him. Recovered from his direct possession and control is one (1) 9mm Beretta pistol
with serial number ABC 12345 and one (1) magazine loaded with 12 live ammos.

2. That we (PStaffSrgt Sabwan S Bukog, PtrlMan Prayd S. Cheken and

PtrlMan Lungag U. Kan-on) immediately arrested the suspect and thereafter informed him of
his constitutional rights in language and dialect known by him in the presence of Barangay
Elected Officials.

3. That we brought the suspect to the Police Station for further investigation and
proper disposition;

4. That we are executing this affidavit/statement to attest the veracity of the

aforementioned facts and to support the filing of appropriate charge/s in Court against the
arrested person.
