Failure to Yield Accident
Rogelio Sanchez, 45 years old, married, lives in Zone 3, Labangal, General Santos City,
reported to the police that on October 3, 2022 about 2:00 o’clock in the afternoon, he
was driving a motorcycle on the highway and was strike his motorcycle by the SUV car
caused of the suspect was fail to proper lookout at the stop light traffic sign . He was
slightly injured and gladly it was not dangerous. After the accident, the suspect refused
to acknowledge his liability on the said accident.

Disposition: This case was filed in court with CC No. 2022-0023, RTC General Santos
City.

SUBJECT : Spot Report re Shooting Incident
DATE : October 15, 2022

1. On or about October 15, 2022 around 8:30 PM, a shooting incident happened
in front of ALMA Residences located in Jose Catolico Sr. Ave, General
Santos City. The victim was identified as Dindo Nicolas, 42, married, and a
resident of Labangal, General Santos City.

2. Initial Investigation revealed that the victim was going to picked up his
daughter when a lone unidentified gunman aboard a Honda Beat 110, color
black, of unknown plate number suddenly stopped and shot the victim five
times in the back. The unidentified suspect fled northwards.

3. The victim Dindo Nicolas who sustained multiple gunshot was rushed to
Socsargen County Hospital but was declared dead upon arrival.

4. The motive behind the incident is still unknown.

2. This pertains to the DOTr Memorandum Circular NO. 2020-119 directing all
sectors of transportation to implement the No Face Shield, No Ride Policy
effective September 29, 2020. Mandatory wearing of face mush be observed
by the passengers along with the other health protocol to ensure everyone’s
safety such as washing of hands, face mask and physical distancing.

3. In this regard, disseminate in your perspective AORs the aforesaid directives


and ensure that wearing face shield will be done by all passengers.

was being robbed by three unidentified persons while the owner was not at
home. The three suspects were being chased by the Security Guard outside
the village but they made good escape and were never identified.

II. Initial Findings:

2. Initial Investigation disclose that the suspects entered at the store in public
market around 11:AM and was seen by the next store owner. The incident
was all shown in the CCTV inside the store. The suspect stab the other
employee and according to the owner, P20,000.oo cash and 10 rim of
cigarettes were missing. He named the culprits as Jelves Bate as his
former employee.

III. FACTS OF THE CASE:


7. On May 29,2022 at 11:AM, Mr. Jelves Bate was seen entering the one
store of Public Market at General Santos City and was seen by the next
store owner. The incident was all shown in the CCTV inside the store. The
suspect stab the other employee and according to the owner, P20,000.00
cash and 10 pack of cigarettes were missing. He named the culprits as
Jelves Bate as his former employee. He was wearing red t-shirt and black
jacket and black pants.

8. The SOCO TEAM led by PCL AURBEY FABE proceed to the crime scene
and they process the crime scene and they found a knife on the floor near
the victim was lying.

9. Initial finding of PSUPT, REYMARK SIERRA, the victims was already


dead for 2 hours and the cause of his death was multiple stabbed.

10. The further investigation confirmed that the victim was identified as Mr.
Antonio Baltazar, 23 years old, single, and a resident at Santiago Blvrd,
General Santos City.
IV. DISCUSSION:
11. According to the initial investigation, Mr. Jelves Bate was fired by his
employer in the reason that he cannot be trusted and hire Mr. Antonio
Baltazar as employee.

V. COCLUSION:
12. Mr. Jelves Bate was jealous of Mr. Antonio Baltazar so he planned to kill
him and robbed the store of his employer.

VI. RECOMMENDATION:
13. Wherefore, in view of the foregoing, the undersigned recommend this
case is closed.
