Discharge Planning

It is a very important part to continue monitoring and provide quality care for Patient X
by following all the health teachings, medication, prescribed diet, treatment and exercise to help
prevent readmission to hospital and increased patient satisfaction.

A. General Condition upon discharge


 Patient’s temperature and heart rate was already in normal range
B. Methods of Approach

Medication

 Patient X was instructed to take medications such as Ferrous Sulfate 1 tab a day
and Folic Acid 1 cap a day.

Exercise

 Advise patient X to follow all the exercises that is being indicated.


Health Teaching
 Advise to take the medication exactly as prescribed.
 Advise patient to drink plenty of water to prevent dehydration
 Advise patient to take plenty of rest to gain sufficient energy
 Do light exercises like walking and climbing stairs and avoid vigorous physical
activities
 Educate patient to take iron supplements an hour before meals for maximum
absorption
 Advise patient not to forget to take the medication and if forgotten, do not take a
double dose of iron the next day
 Advise not to drink milk or caffeine drinks (such as coffee, tea, or cola) at the
same time of taking the medication for faster absorption of medication
Outpatient department (check-up)

 Instruct the patient to return for follow up visit


Diet

Advice the patient to:

 Eat foods high in iron such as red meat, liver, seafood, beans, and dark green
leafy vegetables.
 Eat foods that help the body absorb iron like foods with Vitamin C.
 Eat foods high in folic acid, such as dried beans, dark green leafy vegetables,
wheat germ and orange juice.
