CRITERIA:
o Cleanliness – 20%
o Organization – 30%
o Operation – 20%
o Timeliness of submission of reports – 30%

o Each Department must submit their nominee on or before the end of the week
(November 12, 2021)
o All nominees will be presented on King Henry Team viber account for polling.
o All members of King Henry Team viber group can vote on the list of nominees.
o Poll will be from November 13 – 15, 2021 6PM. No votes will be accepted after 6:01PM
of November 15, 2021.
o The highest vote on the poll will win the following:
o Certificate
o Cash prize amounting to: 3,000.00

PROCEDURE:
o Human Resource Manager will facilitate the process of nomination and judging of
Employee of the Year award.
o Each Department must come-up with their nominees, once they have a nominee they
will answer all the question in the form pertaining to the employee that they’ve
nominated. Once done with the nomination form, it must be submitted to the Human
Resource Department.
o All nominees will be graded by the Department Heads based on the given criteria and
the recommendation from the nomination form without the name of the nominee. The
following are the judges.
o General Manager
o Operations Head
o Accounting Department
o Once the grading is done, the highest scored nominees will be the winner of the
Employee of the Year award.
o If there is a tie, recommendation will be sent to the bosses to determine who the final
awardee will be.
o The highest vote on the poll will win the following:
o Certificate
o Cash prize amounting to: 5,000.00
