SUBJECT : NANYANG—DAMAGED CEILING

This report pertains to the abovementioned event that happened on the ceiling of
Nanyang located the Upper Ground Level Expansion, Phase 3 Area.

Please be informed that on 01 June 2021 around 0028H, night shift on duty, SG Viloria,
Aries of RJC Security discovered a damaged ceiling inside Nanyang Restaurant. The event was
relayed to the undersigned to conduct further investigation. Based on the investigation of the
undersigned, the damaged ceiling may have had caused by a fallen debris from the ongoing
canopy construction in the veranda of the Sunset Terraces. The construction is currently
managed by PAMCOR Constructions

This event was relayed to CRS Supervisor Mr. Chris Denver P. Legaspi and advised the
undersigned to make an Information Report for proper documentation and references.
