SUBJECT : COSTUMER’S SLIPPERS, STUCKED IN THE ESCALATOR

This report pertains to the above-mentioned incident that happened at the downward-
moving escalator of B-1 Level Expansion Area.

Please be informed that at around 1500H of 16 May 2021, a costumer’s Nike slipper
jammed in the downward-moving escalator of B-1 Level Expansion Area. Fortunately, the
costumer didn’t hurt himself. The incident was attended by SG Ong and relayed it to the
undersigned Janzel, They called the attention of Mall Plumber, Mr. Constansio Amoyin to fix the
problem and he was able to remove the slipper from being jammed. They later bought a new
slipper amounting to 199.75 to replace the torn slipper of the costumer.

This event was relayed to CRS Representative, Mr. Ronnie Rick Espiritu and advised the
undersigned to investigate the said incident and make an Information Report.
