History

Presenting complaint (Worthlessness, Anhedonia, Suicidal ideation)

A 23-year-old Chinese gentleman presents with feeling worthlessness since 3 years ago,
feeling depressed and lost in interest to do anything at home and work for the past 2 years,
and intentionally ingesting thinner fluid since the day before admission.

History of Presenting Illness

Patient claims to be having feelings of worthlessness since 2018, which had stemmed from
feeling inferior to his older brother and being treated like a dog by him. He sees his brother
as someone successful in life, able to attain the things the patient wished he had in life, such
as financial prosperity & a CEO position at the company he currently works in. He often gets
downgraded and looked down by his own brother, claiming that he feels as if his brother
views him more as a worker than a sibling. They often get into arguments, of which the
focus of argument revolves around the patient’s unsuccessful life and unwillingness to work
for himself.

Patient states that since 2019, he noticed that he had felt depressed, sad, and gradually lost
interest in doing anything at home, and at work. He says that he grew lazy to help around
the house and would lock himself inside his room whenever his mother nagged him about
not helping around the house. He spent most of his time on his phone, watching videos on
YouTube or playing video games. He remarked the activities as providing him a “kick”
feeling, of which he described to be “a rush of happiness, excitedness, and temporary
escape from life.” He would return to feeling depressed after the activities cease.

He has worked a total of 4 jobs in the past 2 years, citing that he had lost interest to
continue working in each one of them due to the perceived harsh verbal treatment by his
employers which makes him feel demotivated to work. His current job is as the director of
his own e-commerce company where he sells face masks on the Shopee app. He has
employed his ex-girlfriend as a photographer to post pictures of his products on Shopee.
They frequently get into arguments regarding work, and this strains their relationship. He
feels as if his ex-girlfriend does not respect him, even calling his business ideas terrible. He
felt as if no one understands him, including his family and ex-girlfriend.

The day before admission, at around 12a.m, patient claims to have ingested 2 gulps of
thinner fluid, without any prior consumption of alcohol or drugs. The parents had barged in
his room at 12.30a.m after receiving a call from his ex-girlfriend who claims that patient had
called him before ingesting the thinner. They found the patient laying on bed, with thinner
bottle on the ground. He was brought to the ED afterwards, recovered without
complications, and subsequently admitted into the psych ward the next day. When asked on
the reason for ingesting thinner, it was due to his recent breakup with his ex-girlfriend on
that day. He stated that after the breakup, he felt like his life had lost all purpose. He knew
that drinking 2 gulps of thinner fluid would not kill him but he wanted to try it out. He states
that he feels remorseful over his actions but also that he is disappointed for not dying after
ingesting thinner. He is not sure whether he would not repeat this action again in the future.
