JOINT AFFIDAVIT

WE, ____________________________ and __________________________,

both of legal age, Filipinos, with residence at Victorias City, Negros Occidental, after
first being duly sworn according to law, depose and say:

1. That we personally know the spouses RAMONITO H. BAÑARIA and

RENELDA ABAY who are residents of Victorias City, Negros Occidental;

2. That the marriage of said spouses was solemnized at the Roman Catholic
Church of Victorias, Victorias City, Negros Occidental by Rev. Fr. Arecio P.
Dormido on July 16, 1998, but was not registered by the Office of the Civil
Registrar of said place during its reglementary period;

3. That we know this for a fact because we are friends with the above-named
spouses;

4. That we are executing this affidavit in order to state the fact of the marriage of
the said spouses whose marriage was not registered with the Office of the
Civil Registrar of Victorias City, Negros Occidental due to ignorance of the
requirements of the law and to avail for the late registration of their marriage.
