AFFIDAVIT OF LOSS

I, LOREN A. JANAGAP, of legal age, Filipino, with residence at

Victorias City, Negros Occidental, after first being duly sworn according to law,
depose and say:

1. That I am the authorized representative of owner of NJJR STORE,

a grocery store located in the ground floor of Victorias Commercial
Center;

2. That as the authorized representative of said business, I have in my

custody the following documents:

a. Petty cash notebooks

b. Cash receipts up to December 31,2020
c. Cash disbursment books
d. Cash sales books
e. All BIR tax returns files up 2020.

3. That the said documents were lost during the flash floods last
January 1 and 9, 2021 and despite repeated efforts to locate the same,
the same could no longer be found or located and are already lost and
gone;

4. That I am executing this affidavit in order to state the fact of the

loss of the said documents.

1. That I am the owner of JASMINE NICOLE (JN’s) Coffeeshop, a coffee

shop located in the Food Court of Victorias Commercial Center;

2. That as the owner of said business, I have in my custody the following

documents:

c. Cash disbursment books
d. Cash sales books
e. All BIR tax returns files up 2020.

5. That the said documents were lost during the flash floods last
