Intern Program Application Instructions - Part 2

MANDATORY SUPPORTING DOCUMENTS


Please submit the following supporting documents via email to mkitchin@amity.org as soon
as possible. Visa paperwork cannot be issued until all documents have been submitted and
approved. When submitting your documents, label each attachment appropriately.
All documents must be written in English or accompanied by an English translation.

Two Professional References


References should be completed by someone who knows you in a professional capacity (such
as a professor or employer). References must be signed.

Health Evaluation Form


The Amity Health Evaluation Form must be completed and signed by you and by a certified
physician. Please answer every question carefully and honestly. This form will be kept in our
files as well as sent to the Host School and Host Family to assist the applicant in case of a
medical emergency.

Health Memo of Understanding (MOU)


This document confirms that you acknowledge the information provided within.

Financial Support Statement/Confirmation of funds


Interns are required to have sufficient funds to pay for the following program-related costs:
• SEVIS fee ($220)
• Visa application fee (approx. $160)
• Travel expenses (varies based on time of year, destination, etc.)
• Health insurance (approx. $95 per month; must be paid in full prior to arrival in the U.S.)
• Personal spending money ($250 per month for the duration of their assignment)
You may add a statement from a sponsor or family member to provide verification of sufficient
funds. Please complete the top portion of the Amity Financial Support Statement Form and have
your financial institution or the financial institution of your sponsor complete the bottom portion
of the document.
Note: If your bank is not able to complete the bottom portion of the document, please provide
any official document confirming yours or your financial sponsor’s funds.

***You must show proof of US$2000 during admissions, although we strongly recommend
that you have at least $2500 available at the time of your arrival in the U.S.***

Criminal background check
All applicants are required to submit an official criminal background check, usually issued by
your local authorities.

Verification of studies
Please submit proof of your current university enrollment, including the dates of the enrollment
period and proof of full-time status. If you have graduated, please send us a copy of your
diploma.

Scanned copy of valid passport


Amity must receive a clear and readable copy of your passport to create your visa documents.
Your passport should be valid beyond your intended period of stay in the U.S.

English Proficiency Test


You must submit your results from a formal English Proficiency Test or other certification that confirms
your level of English, preferably on the CEFR scale.

Screening Interview
To complete the application process, you must pass a screening interview with an Amity Screening
Officer either in person (if applicable) or via Skype or phone. Once you have submitted the documents
listed above, you will be assigned to a Screening Officer and your interview will be scheduled. Please
note that a non-refundable screening fee may be required at the time of the interview.
