QUITCLAIM AND RELEASE

KNOW ALL MEN BY THESE PRESENTS:

That I, _______________________, of legal age, Filipino and with  address at Sandiat

West, San Manuel, Isabela, in consideration of PESOS: Three Thousand Eight
Hundred Thirty-Three Pesos Only(P3,833.00), received by me in full and to my entire
satisfaction for all moneys and benefits, specifically, separation pay, due me in
connection with my past employment at ___________________________, hereby
release and forever discharge ____________________________ including all its
officers, from any and all money claims, causes of action  or otherwise.

 That, I have no more claims or rights of action of whatever kind and nature against the
said Company and any officers thereof.

 And I hereby confirm that this Release and Quitclaim is being executed by me
voluntarily and in a language familiar to me and is not intended to circumvent or defeat
any provision of law, rule or regulation.

 That, I hereby attest that I fully understood all the terms and conditions of this Release
and Quitclaim as well as the legal implications thereof and I hereby express my
complete agreement thereto.

IN WITNESS THEREOF, I hereunto affix my signature this 15 th of June, 2021 in the City
of Ilagan.
