CONDUCT OF THE NOVEMBER 2021 PHARMACISTS LICENSURE EXAMINATION

The Professional Regulation Commission (PRC) announces the conduct of the


November 25 and 26, 2021 Pharmacists Licensure Examination (PhLE) in Baguio City
to 238 examinees. The testing venue is at the University of Baguio, General Luna Road.

All examinees are required to accomplish the entry record sent to their email addresses
or posted at www.facebook.com/prc.cordillera for monitoring and contract tracing
purposes. Examinees coming OUTSIDE of Baguio, La Trinidad, Itogon, Sablan, Tuba
and Tublay shall strictly adhere to the latest entry protocols of Baguio City as posted
www.facebook.com/pio.baguio such as the presentation of the following at the Baguio
City Central Triage:

1. If fully vaccinated in a legitimate facility outside of Baguio:


a. QR Code issued through a duly registered Health Declaration Form at
https://hdf.baguio.gov.ph;
b. COVID-19 domestic Vaccination Certificate issued by a legitimate vaccinating
establishment or International Certificate of Vaccination issued by the Bureau of
Quarantine; and
c. Notice of Admission (NOA) or Room Assignment as proof of indispensable
transaction.

2. If not fully vaccinated:


a. QR Code issued through a duly registered Health Declaration Form at
https://hdf.baguio.gov.ph;
b. NOA or Room Assignment as proof of indispensable transaction; and
c. Negative results RT-PCR test conducted within seventy-two (72) hours prior to
entry into Baguio.

At the testing venue, examinees shall bring/submit the following:


1. NOA;
2. Ballpens and pencils;
3. Photocopy of the complete Vaccination Card if fully vaccinated; or Negative
result of the RT-PCR test (NOT Antigen test), date of collection shall be within
the period November 20 to 24; or Certificate of Mandatory Quarantine for 14
days that shall end within the period November 20 to 24, or its equivalent signed
by any of the following:

a. Any licensed Physician (government or private physician)


b. Municipal Health Officer, Provincial Health Officer or City Health Officer
c. Barangay Officer visiting the examinee’s residence attesting that he/she has
been in quarantine for 14 days

4. Duly accomplished Informed Consent Form and Health Declaration Checklist


(Annexes A and B of Memorandum Order No. 68 downloadable at
www.prc.gov.ph);
5. N95 or KN95 face mask and face shield to be worn at all times;
6. 70% ethyl alcohol for regular hand disinfection; and
7. Pre-packed meal/snacks and drinks.

Refer to the program of examination for other allowed and prohibited items.

Examinees shall submit the Post Examination Health Surveillance Form (Annex C of
Memorandum Order No. 68) to car.examination@prc.gov.ph 15 days after the
examination or on December 11, 2021.

An examinee shall be allowed to defer taking the licensure examination provided that
the following documents are submitted on or before November 10, 2021:
1. Written request duly signed by examinee; and
2. Photocopy or electronic copy of the NOA.

Transfer to another testing center SHALL NO LONGER BE ALLOWED.

In the event of failure to take the November 2021 PhLE, examinees may submit a
written request with the required supporting documents on or before December 3, and
shall be allowed to take the April 2022 PhLE subject to the following valid reasons:
a. For health-related reasons – shall be supported with a medical certificate;
b. For quarantine-related travel restrictions – shall be supported with an official
certificate of border/travel restrictions issued by the concerned local government
unit.

The written request and supporting documents shall be submitted through email at
car.application@prc.gov.ph or car@prc.gov.ph. Said examinees are advised to keep
their Notice of Admission for use during the next examination.

Examinees and examination personnel are reminded to strictly observe the minimum
health standards set by the Department of Health and to comply with the provisions of
PRC Memorandum Order No. 2020-68 dated November 06, 2020 re: Updated Standard
Guidelines on the Strict Health Protocols to be Observed in the Conduct of Licensure
Examinations During the COVID-19 pandemic for everyone’s safety, and to immediately
go home after the conduct of the examination. With Baguio’s weather condition,
examinees and examination personnel are advised to use additional layer of clothes,
bring rain gears, and flashlights in case of power outage at the testing venue.

The Commission expresses its gratitude to the Cordillera RIATF and RTF, officials and
staff of the City Government of Baguio, Bureau of Fire Protection, Baguio City Police
Office, University of Baguio, and examination personnel for their unwavering support
and cooperation.

GENERAL INSTRUCTIONS:

1. Check or verify your school/building assignments at the PRC official website


(www.prc.gov.ph) or through the Notices/Announcements to be posted at the PRC
premises two (2) to three (3) working days before the date of examination.

2. Report to the school/building assignment before 5:30 a.m. on the first day of
examination to verify your room and seat number. Be punctual. Late examinees will not
be admitted.
3. Examinees are required to wear the following attire every examination day:

1. Male Examinees - White polo shirt with collar (without any seal, logo or mark)
Decent pants or slacks
2. Female Examinees - White blouse or shirt with collar (without any seal, logo or mark)
Decent skirt or slacks

4. Bring the following on examination day:

a. Notice of Admission
b. Official Receipt
c. One (1) piece metered-stamped window mailing envelope
d. Two or more pencils (No. 2)
e. Ball pens with BLACK ink only
f. One (1) piece long brown envelope
g. One (1) piece long transparent/plastic envelope (for keeping your valuables and
other allowed items)
h. Health Forms (Pursuant to Memorandum No. 68, series of 2020)
i. Negative RT-PCR Test Results, if applicable, or Certificate of Quarantine
(Pursuant to Memorandum No. 68, series of 2020)

5. The following are PROHIBITED inside the examination premises/rooms.

a. Books, notes, review materials, and other printed materials containing coded
data/information/formula.
b. Calculators which are programmable or with embedded functions especially
CASIO FX991ES and CASIO FX-991ES plus.
c. APPLE, SAMSUNG AND OTHER SMART WATCHES, CELLULAR PHONES,
BLUETOOTH, EAR PLUGS, TRANSMITTER, PORTABLE COMPUTERS AND
OTHER ELECTRONICS GADGETS/DEVICES.
d. Bags of any kind (ladies bag, shoulder bags, attaché case, backpacks, etc.)
e. Other examination aides not stated on this program.

6. Per Memorandum Order No. 24 (s 2020)1, the following precautionary measures and
protocols shall be observed and implemented during the conduct of licensure
examination such as, but not limited to:
i. All examinees shall be required to wear and bring protective face mask 2 and
alcohol-based sanitizers. For identity verification purposes and pre-examination
checking of examination paraphernalia, the face mask shall be required to be
properly taken off temporarily to show the examinees’ face and the inner part of
the facemask. Face masks during the progress of the exam shall not be allowed
to be taken off.
ii. All examination personnel shall be provided with extra protective face masks or
face shield and latex gloves.
iii. All examinees shall be required to queue upon entry with a one-meter distance
radius from other examinees.
iv. Body temperature shall be taken through thermal scanners prior entry to the
testing venue premises. Those with fever, cough or colds shall not be allowed
entry to the testing venues. Affected examinees shall be allowed, however, to
take the next licensure exam without additional cost.
v. Physical/social distancing among and between examinees and examination
personnel during the entirety of the exam and during their entire stay at the
testing venue shall be observed. Congregation, gathering or assembly within the
examination rooms, hallways or any area within the testing venue shall be strictly
prohibited.
vi. All examinees shall not be allowed to go outside the examination room during
break time and lunch time. All are advised to bring own packed lunch and snacks
to be placed under the seats, and to be taken out only during break time.

vii. Proper hand hygiene shall be observed particularly every after use of the
restrooms.
viii. Prior to entry to the examination room, examinees shall be required to sanitize
their hands and their plastic envelopes with their sanitizers or alcohol.
ix. Prior to the distribution of the Test Questionnaires/Booklet, examinees shall
again be required to sanitize their hands using their alcohol or hand sanitizers.
Hand sanitizers thereafter shall be placed in the examinees transparent
envelopes and shall be deposited or placed at the designated area of the
examination room. The same process shall be done every start of each
examination subject.
x. The testing venues shall be required to conduct thermal scanning procedure
including the provision of sanitizing footbath and for their security personnel to
implement proper COVID-19 precautionary measures.

7. In addition to the above, pursuant to Memorandum No. 68 (s. 2020), the examinees are
required to submit RT-PCR Test Results, if falling under the groups stated in DOH
Memorandum 2020-258A, or Certificate of Quarantine. Only those examinees with
negative results shall be allowed to take the examination. Please see this link

8. Read carefully and follow the instructions on your NOTICE OF ADMISSION and
EXAMINEES GUIDE.
