Mantri Park Vaccination Drive FAQs:

1. How do I register for this society drive?

There are 3 steps you need to do in order to register yourself:
a. Register yourself on the COWIN portal or the app with your mobile number
b. Fill in your details in the Google Forms link shared here and enter the same
mobile number that you registered with on the COWIN app / portal
c. You will need to pay for the vaccine with in 2 days of you filling up the form.

2. How will I know if I am eligible for the vaccine?

If you are above 18 years of age as per your Aadhaar Card, you are eligible to sign up
for the Mantri Park Vaccination Drive

3. Which vaccine will I get?

At the moment, the vaccination centre has got a confirmation to give Covishield only

4. Which is the vaccination centre / hospital that we have tied up with for the
vaccination drive?
We have tied up with INVC, one of India’s leading vaccination centre. You can get
more information on the vaccination centre (VC) from here - India Vaccination Centre
- INVC - Apna Family Vaccination Center. They are authorised by PCMC (Thane
Municipal Corporation to carry our vaccination drives across India).

5. How much do I need to pay for the vaccine?

You will need to pay Rs. 999 + 5% GST for the vaccine and Rs. 200 + 18% GST as
administration charges to the society without which you will not be able to get the
jab.

6. I have not got any dose of the COVID19 vaccination. Am I eligible?

Yes, you are eligible

7. I have taken the 1st dose. Am I eligible?

If the gap between the 1st dose and the 2nd dose is 84 days and more, you are eligible.

8. When is the drive scheduled for?

The vaccination drive is scheduled tentatively for the 2nd week of June starting from
June 7, 2021 basis availability from the service provider.
9. Till when do I need to pay?
You will need to pay within 2 days of filling the Google Form.

10. What arrangements are being made in order to follow protocols for the
vaccination?
We are complying with all protocols that have been advised i.e. we will allow only 20
people per slot to maintain social distancing and over crowding. We will have 1
dedicated area for the vaccination, 1 area for observation and 1 area for waiting for
30 minutes.

11. What precautions will be taken by INVC and what level of readiness will we have in
order to handle any untoward incidents?
The INVC team will have a 1 doctor, 2 nurses, 2 data entry operators and an
ambulance with all the necessary equipment to handle any emergencies.
INVC also has tie ups with hospitals nearby to handle any situation that needs
immediate medical attention.

12. Till when do I need to fill the Google Forms?

You will be required to fill the Google Forms by May 24, 2021. We will then initiate
the next step of fixing a date for the vaccination.

13. Can I get my family members / house help / servant / nanny for the vaccination
drive?
Yes, you can as long as you fill in their details and make the necessary payment on
their behalf to the society

14. Where is INVC getting their vaccines from?

INVC is procuring the vaccines from Serum Institute of India, one of the official
companies producing the vaccine.

15. How will I get my vaccination certificate?

Your certificate will be generated post getting the vaccination via an SMS that you
will get from the regulatory body, post which you can log in to the COWIN app /
portal to download it.

16. Will my data be shared with INVC?

 You will be required to submit your mobile number to INVC at the time of the
vaccination and carry your Aadhaar Card to prove your identity. Besides that, no
other data of yours will be shared with INVC.

17. What is the role of Mantri Park Co-operative Housing Society Limited?
Mantri Park CHSL is only facilitating the vaccination drive by tying up with INVC and
by inviting them to the society premises to discharge the vaccinations. Mantri Park
CHSL will not be responsible for any adverse effects, if any. The role of MPCHSL is
limited to liaising with INVC and making the vaccination drive feasible.

18. Will MPCHSL also facilitate the 2nd vaccination dose?

Yes, as far as we have enough members for inoculation and basis availability from the
service provider.
