1.0 PURPOSE
The purpose of this Country Crisis Response Plan is to establish guidelines and
a structure for the effective handling of foreseeable emergencies and to ensure

that all employees are aware of their duties and responsibilities in an


emergency situation throughout Weatherford operation in Bangladesh.
An emergency situation is defined as an occurrence or series of occurrences
which involve the health and safety of the workers or public through
uncontrolled fire, serious injury, health failure or other threat to the people or
the environment.
During any emergency, full coordination with Chevron will be arranged and the
current rig ERP will marry Chevron response plans.
This Plan defines the chain of command and associated responsibilities for key
people. It also contains a description of the response/decision making process
in the form of flow charts and a list of emergency contact details.

2.0 SCOPE
2.1 This Country Crisis Response Plan is applicable to all Weatherford operations in
Bangladesh. For rig specific operations, the rig Emergency Response Plan (ERP) is
to be used as the reference document.

3.0 RESPONSIBILITIES
The Project Manager shall ensure the management, implementation and review
of this plan.
The country QHSSE Manager is responsible for updating this plan.
Department Heads should provide input during drill exercises within their area
of responsibility. This includes findings, feedback and recommendations for the
purpose of ensuring the effectiveness and continual improvement of this plan.

4.0 PLAN

4.1 Fire/explosion emergency

4.1.1 Fire/explosion emergency at rig site


Refer to rig specific emergency response plan.

4.1.2 Fire/explosion emergency at office or support base


All office and support base personnel will be trained as minimum to basic fire
fighting level. A minimum number of selected personnel will be trained to

advanced fire fighting level, including practical exercises on the use of the fire
fighting equipment available at their work site.
A minimum of two employees per worksite will be nominated as Fire Wardens.
Fire Wardens will be trained to advanced fire fighting level and will be
responsible for coordinating the mustering, headcount and evacuation
procedure. They will also be responsible for contacting local governmental
services such as fire department and medical services in case of a fire
emergency at their worksite.
Each worksite will have a specific fire plan posted in appropriate areas where
all employees and visitors have access. The plan should provide a layout of the
worksite including fire fighting equipment type and location, fire alarm activation
points, evacuation pathways, muster point and emergency contact numbers.
It is the responsibility of the worksite Manager at each office and support base
to ensure that the site specific fire plan is up to date and is attached as
appendix G to this CCRP.
The following are the precautions that must be learnt and followed whenever a
fire is discovered.

 When fire is discovered

1. Raise alarm: shout “FIRE! FIRE!! FIRE!!! or sound Fire Alarm


2. Announce the location of fire
3. Attempt to extinguish the fire if safe to do so
4. Ensure your line of escape is not threatened or blocked
5. Never turn your back on a fire

 When fighting fire

1. Stay upwind of the fire


2. Watch out for wind changes
3. Watch out for flashbacks
4. Watch out for spot fires
5. Be alert for contaminated air / atmosphere
6. Be alert for electrical hazards and structural damage
7. Never open a heated door or enter a fire scene without a charged fire
hose

8. Monitor the fire after extinguished to ensure it is out


9. Clean and secure the scene after a fire
10. Never use water or foam on electrical fires until power is switched off
11. Never use jet stream water (fire hose) on flammable liquid fires as it will
spread the fire and may cause flashbacks.

4.1.2.1 Office and support base fire emergency flowchart

4.1.2.2 Drills & Exercises


A fire/muster drill should be conducted on each office/support base location at
least quarterly. Records should be kept using the appropriate BMS forms
including recommendations for improvements.

4.1.3 Personnel evacuation from office and support base


See appendix B

4.2 Bomb threat at office or support base


In case of a bomb threat, all personnel should immediately evacuate the
worksite. For this specific hazard the existing muster point may be unsuitable.
The most senior personnel on site should consider moving personnel to an
area away from the worksite to minimize risks of injuries. Local police and fire
department shall be contacted as soon as practicable. Personnel should not
return to worksite until they receive “all clear” sign from the person in charge.
After normal operations resume, the Project Manager should prepare a report
and submit to region office and Corporate Security Manager.

4.3 Hazardous material (Hazmat) spill emergency

4.3.1 Hazmat spill at rig site


Refer to rig specific emergency response plan.

4.3.2 Hazmat spill at support office and base


The appropriate storage and handling of hazardous materials is the key to
proactive hazmat spill prevention. Where applicable, hazardous materials shall
be stored in a well ventilated area away from frequent personnel or equipment

movements. Special considerations include, but are not limited to, secondary
containment, storage compatibility with other chemicals and proper signage
shall be in place to minimize fire and spill risks.
If a spill does occur, we should follow the steps detailed in the following section.

4.3.3 Spill control counter measures


1. Contain spill and close or isolate the spill source
2. Notify the person in charge immediately
3. Immediate line supervisor, organize to respond as necessary with spill
control equipment, including absorbent, bonding, skimmer etc or use
improvised containment equipment
4. Dispose clean up materials as per local requirement/regulation

4.3.4 Hazmat spill emergency flow chart

4.4 Injury / illness emergency


Weatherford shall coordinate with Chevron health and medical services which
will assist on any required patient evacuation to one of the hospitals in Dhaka.
Evacuation from Bangladesh will be the responsibility of Weatherford.

3. Assess the hazards at the scene. Make the area secure and safe if an
accident has occurred. Switch off electricity if electricity is involved. Protect
yourself first. Don’t become a second victim.

4. Find out what happened.

5. If head or spinal injuries are suspected, support the head and neck.

6. Assess responsiveness. If there are hazards, serious injuries or casualty is


unresponsive call an ambulance immediately.

7. Check airway, check breathing, and check circulation.

8. Start CPR or rescue breathing if necessary and if you know the proper
technique (2 rescue breath X 30 chest compressions).

9. Do not be hurried into moving an injured person; be sure that a thorough


examination has been made for all injuries and that they are properly
protected by dressings, splints etc. The paramedic team will decide on the
method of evacuation.

10. If a severely injured person must be moved, he/she should always be


transported laid down.

11. Always suspect a back injury in a seriously injured person and make every
attempt to keep the back straight if he/she must be moved. Hold tension on
the victim’s neck and head and move the entire body as a unit.

4.4.2.2 Medical incident flow chart


The medical incident flowchart (appendix D of this crisis plan) is to be followed
in case of medical emergency.

4.4.2.3 Information about injured person


In most cases when incidents are reported to insurance companies, medevac
call center or persons in charge within Weatherford, a minimum amount of
information is required about the injured/ill person. The form provided in

appendix E will help the person reporting the incident to gather the necessary
details.

4.5 Urgent medical evacuation for critical illness / injury


In the event an employee sustains a critical injury or illness that requires urgent
treatment not locally available, medical evacuation by air ambulance may be
authorized. Weatherford is a member of MEDEX Assistance Corporation. The
services offered by this group are an international air ambulance evacuation
capability and a 24 hours phone link medical appraisal and consultation
service.
If the determination is made that one of these services is required, obtain the
Project Manager’s approval and follow the procedure outlined below.
It may be deemed necessary to use the MEDEX phone link medical appraisal
service in any situation where there is no definite diagnosis of the injury or
illness or the Project Manager believes further consultation may be required.
If the MEDEX air ambulance evacuation service is to be used, consideration
will be given to the transportation of the patient within Bangladesh to an airport
suitable for immediate and/or 24hr use by MEDEX. These facilities will be
identified by MEDEX following the call.
The Project Manager or his alternate should contact a Weatherford MEDEX
authorized person (AP) in regional office (Vice President). AP will contact the
following telephone numbers in the event that emergency service is required:

The Personnel Manager assigns a Government Relations Representative to co-
ordinate the activities itemized below:
 The nearest police station is informed about the death. The police
investigate the cause of death, and the body is brought to the morgue (If the
body needs to be transported by air from remote location, death certificate
shall be handed to the pilot before departure).
 The police prepare a primary report describing the circumstances
surrounding the death. Forensic prepares a death report based on the post-
mortem examination.
 The reports are certified by the police and taken to the Ministry of Health.
 A death certificate in Bangladesh is issued by the Ministry of Health.
 The police report and the death certificate are translated in the language of
the deceased.
 The translations are attested by the Ministry of Foreign Affairs and from the
deceased employee's Embassy.

 The relevant Embassy issues a letter of no objection to release and prepare


the body for embalming.
 Clearance is granted by the Police.
 Forensic authorizes release.
 An official request for embalming is completed and submitted to the Ministry
of Health, together with the following documents:
 Copy of passport particulars including the Residence Permit Stamp
page.
 Copy of Civil ID.
 Original death certificate.
 Copy of attested death certificate.
 Arrangements are made with a licensed Mortician to perform the embalming
at the Ministry of Health.
 The Mortician issues a certificate of embalming.
 A coffin of the appropriate size is made available with a suitable label
bearing the deceased's name and the destination address.
 Twenty-four hours prior to shipment of the body, an appointment is made for
a doctor from the Ministry of Health and a representative from the deceased
embassy to witness the placement of the body inside the coffin. The coffin is
closed and sealed in the presence of both witnesses by a welder who is
brought to the Ministry of Health at least two hours before the appointed
time.
 The Government Relations Representative is responsible for arranging the
cancellation of the deceased's residence permit.
 Shipment protocols that should be observed with the Immigration, Cargo,
and Custom authorities, making available:
 5 copies - Death Certificate
 5 copies - Mortician Certificate
 Original passport of the deceased

 Expeditious processing of any financial benefits due should the deceased


be an employee.
 Inventory and packing of the deceased employee's personal properties in
co-ordination with representatives from the Warehouse and Safety
departments.
 Completion of documents that will be required if a personal escort is
designated by the company to accompany the sealed coffin to its
destination.
 Transport of the sealed coffin to the airport, using an official vehicle
provided by the Ministry of Health.
 File copies of the documents related to the employee's death and
disposition are forwarded to Personnel Administration at the conclusion of
the above activities.

4.7 Road accident emergency

In the event of a vehicle incident, follow the rules outlined below:

a. Do not move your vehicle from the scene of the incident.


b. Wait at the site of the accident until the police arrive and note the plate number
of the other vehicle.
c. After arrival at the place of the accident, the policeman will take brief details of
the accident and inspect the driving license and car registration book from both
drivers.
d. With the permission of the policeman, you may remove the car from the place
of the accident.
e. You will be asked to report to the local police station to complete the accident
report in the presence of an investigating officer.
f. If the accident occurs AFTER company hours:

o Go to the police station with the policeman and complete the


investigation.
o Contact the designated Company representative for assistance if
needed.

o Driver may be required to leave the car registration book and driving
license at the police station until investigation is completed.
o If the car cannot be moved after the accident due to damage or technical
fault, Weatherford office will arrange to tow the vehicle to the garage,
after obtaining a police report.

g. If the accident occurs DURING company hours:

o Call the office and advice the Weatherford HR Department of the


accident, where it took place and the name of the police station where
you are located.

h. If the driver is not seriously injured, but requires medical treatment, ambulance
will take him to the nearest government clinic. Driver will then be required to
attend the police station to complete the accident report.

Company vehicles carry comprehensive insurance cover. However, a police


report must be obtained to enable the vehicle to be repaired under the
insurance cover.

FOR THE INSURANCE COMPANY TO PAY FOR REPAIRS TO A VEHICLE,


A POLICE REPORT MUST BE OBTAINED. THIS IS REQUIRED WHETHER
THE CAR IS DAMAGED IN A MOVING OR NON-MOVING ACCIDENT.

4.8 Evacuation of expatriate employees and dependants due to civil


unrest or natural disasters
Weatherford project Manager and QHSSE Manger will receive daily notification
from the SOS website and whenever a security or weather hazardous
conditions arises as defined in the Country Security Manager responsibilities’
handbook. These early notifications will assist Weatherford on planning any
necessary evacuations effort well in advance.
Continuous information sharing and cooperation with Chevron will be very
important to handle security and weather threats.

4.8.1 Authority to act


The decision to implement these procedures will be made by the Project
Manager through communications with Region office and corporate Security
office.
The decision to carry out an early, partial, or full evacuation will normally be
announced by the Project Manager after consultation with the region office and
site managers, Client(s) and, if possible, the Embassies.
In rapid-developing life-threatening situations, which preclude prior consultation
with appropriate company and government representatives, the senior
employee on the scene at the time has the authority to make the decision to
evacuate.
All security related actions should be in compliance with Country and regional
security policies.

4.8.2 Information rosters


The HR representative should provide the Project Manager with an up to date
list of the following information:
 List of all expatriate employees and dependants with full contact details
 List of employees and dependants residences including contact
numbers, addresses and any other location frequently used such as

Situation: Local political disturbance or sporadic terrorist acts under control of

personnel to maintain contact with office and cease all unnecessary travel

Level 3 (Button Down)

Situation: Local political disturbance on a continuing basis where it appears


to be under control of local authorities.
Action: Dependents who have been expatriated to Bangladesh by the
Company leave voluntarily, at Company expense. Employees who opt to
depart will be treated in the same manner as an employee who does not
complete a contract.

Level 3A
Situation: Same as Level 3 situation with the exception that local management
has deemed it necessary to evacuate dependants.
Action: Dependants who have been expatriated to Bangladesh by the
Company must leave, at Company expense.
Depending upon circumstances either:
(a) Employees are requested to remain in Bangladesh; however, if they elect to
leave they will be treated in the same manner as employees who have not
completed their contract, OR
(b) Employees have the option to depart with their dependants. Those
employees will be treated as if they had completed their contract
assignment.

Level 4 (Partial Evacuation-Trickle Evacuation)

Situation: Local political disturbance on a continuous basis where the local


authorities appear to be losing control.
Action: Non-essential expatriate employees and dependents depart without
option. Non-essential local hires and their dependents leave at their option.

Situation: A potential unfriendly takeover of local government. Operations are


shut down and facilities are secured. Wells should be secured in the best way.
Rigs should be powered down and disabled electronically.
Action: Essential expatriate employees and dependants depart.
Essential local hires and their dependents depart at their option.

4.8.5 Rig evacuation

1. In the event of Project Manager decision for full evacuation, he will notify
of the emergency and determine appropriate strategy

2. Notify the Operation Managers of all product lines for wells and
equipment to be secured

3. Operations Managers will notify their Supervisors for shut in operation,


securing the well and power down; prepare transport to Weatherford
office.

4. Rig crew and service hands will be transported to Weatherford office by


rig car/bus/boat/ferry launch/plane.

Notes:

1. Project Manager in Level 3 may elect to evacuate expatriate personnel


only and senior national personnel will stay to run the location.

2. Project Manager, in discussion with relevant authorities and regional


management, will define the evacuation route(s).

3. H.R. Manager shall prepare all relevant documents and keep ready for
critical cases.

4. Personnel and their dependants under mass evacuation may be limited


to one piece of hand carried baggage per person.

5. All personnel should keep travel documents such as passports


immediately available, as well as any critical personal needs and
medication.

6. Partial evacuation may be carried out through the normal commercial


transport carriers or chartered transport services.

7. H.R. Manager and Finance Manager should prepare air tickets and other
transport means for national personnel, when instructed by Project
Manager.

8. Finance Manager will prepare the cost of evacuation for expatriate and
national employees separately and furnish copy to Project Manager.

9. All personnel should only bring with them necessary items in the event of
evacuation.

10. All personnel shall prepare their rooms for evacuation, secure all
valuables and carry with them only necessary items.

4.8.6 Critical tasks


When situations arise (Levels 3, 4, 5) which seriously threaten our employees,
the critical tasks are to:
(a) Notify all personnel of the existence, nature and extent of the threat (Level 1
- ALERT).
(b) Advise all personnel and dependents to minimize travel from office,
worksites or home (Level 2 - STAND FAST).
(c) Assemble all personnel in local office or other nominated complex (Level 3 -
PARTIAL EVACUATION).

(d) If necessary transport all personnel from the danger area to a safe haven
utilizing commercial airlines, or charter service.
(e) Contact air charter service to schedule if commercial carriers are
unavailable.

4.8.7 Administration and logistics

i. Accounting for Personnel


(a) A most demanding, and often exasperating task in an emergency is the
precise accounting of personnel. During any evacuation, every employee
must be accounted for from the time the crisis develops until the person
and/or his dependants have returned to his point of origin. Up until the time
personnel are safely aboard carriers and moving away from the danger
area, authorities place particular emphasis on the precise identification and
location of individuals. Further, they often demand personnel rosters which
require additional data, such as nationality, sex, age and passport numbers,
on a single form.
(b) The H.R. Manager shall maintain a current listing of all Weatherford
employees and their dependants, to include basic personal data, on a
computer program so as to permit speed and flexibility in the preparation of
personnel rosters. A hard copy of a weekly updated roster will be at the
office and his home.

ii. Assembly Areas

(a) The primary assembly area for personnel in the Weatherford office is the
office building. The secondary assembly area for the rest of personnel is the
Weatherford office ground floor.
iii. Transportation
(a) Personnel in the Weatherford office will use presently assigned
transportation to move to assembly areas, or to air or sea embarkation
points, when directed to move. The rigs will use the vehicles assigned to
them.
(b) It shall be standard procedure for personnel to keep their vehicle fuel tanks
no less than half full at all times with a case of bottled water in the trunk.

(c) The Warehouse shall store (10) sets of emergency supplies, such as
flashlights and batteries, gloves, first aid kits, rope and basic auto tools.
iv. Accompanying Baggage
There are no baggage handlers during emergency evacuation. Therefore, each
individual will be limited to one piece of hand carried baggage, and pets will be
prohibited.
v. Control Centre
The Weatherford office will be the principal ‘Communications Center’ for the
Project Manager.

5.0 Revision requirements


This Plan shall be reviewed and updated annually by the Project Manager
during the fourth quarter of the calendar year.

2. Switch off any potential source of ignition
3. Make area safe to prevent further escalation
4. Provide medical assistance, if required
5. Verify spill source, record details and identify corresponding MSDS
6. Attempt to control spill with recommended inert material
7. Continuously report by radio/telephone on situation
