STATEMENT OF THE CASE

Mr. Moises Divina Gracia Sr. 64 years old and was born in December 18, 1957.
Born and raised in Argao, Cebu City, he moved to kalubihan quiot as he start
having his own family, Mr. Moises was diagnosed with High Blood
(Hypertension) that may cause him stroke. He owns a business meat shop at
Carbon with his wife until now, Because of his business is able to support his
family and all of their needs.
Mr. Moises had 4 children and 4 of them graduated in the same medical courses.
He was a workaholic man and at the same time eating unhealthy foods, he states he
is an alcoholic. Patient’s state he used to be active and managed it business alone.
He didn’t realize that abusing his body and living an unhealthy lifestyle would
affect his blood pressure so much. His family history in medical aspects had a
diabetic and high blood, he also told me that he had diabetic too and doctor
prescribe him for maintenance but he told me that he most concerns a lot is his
high blood and worried not to recover.
He told me that he knew that he had high blood at the age of 50 but he wasn’t
careful enough that time and be abusive by eating unhealthily and no exercise he
just sits and do nothing aside he is managing his business before that happen to
him.
Mr. Moises told me that he was in the ICU 1 week back in 2017 he remembered
that he calls his son that he better go to the doctor for checkup because he was
having a color blindness, short breathing and dizziness and that was the start that
he got heart attack.
