responsibilities and etc.
ORGANIZATION. The ability to plan and organize work effectively.

INITIATIVE. The extent to which the trainee is a self-starter in attaining the objectives of his job.
INDUSTRY. The extent to which the trainee may describe as a hard worker and the amount of concentration and effort
in the performance of his job.
ATTAINEMENT. The extent to which the trainee successfully accomplishes the task or functions assigned and the
delivery of the desired results.

ATTITUDE TOWARD WORK AND THE ESTABLISHMENT. The nature of the trainee's feelings about the
establishment and interest in the job.
ATTENDANCE. Nature of the trainee's feelings toward time and punctuality.
COOPERATION. The extent of the trainee's cooperation with the employees of the establishment including the ability
to act with supervisors of colleagues for the benefit of the establishment
PHYSICAL ATTRIBUTES. Personal cleanliness, poise, proper attire and general bearing.

COMMUNICATION. Ability to express as well as comprehend ideas both orally and in writing.

CHEMICAL HANDLING . The student was able to apply all the skills learned in proper chemical handling. E.g. Use of
MSDS, dilution, chemical use, etc.
CLEANING . The student was able to apply all the skills learned in proper housekeeping. E.g. apply proper cleaning
procedures for different areas; cleaning and maintenance of the cleaning materials and equipment, etc.
BED MAKING . The student was able to effectively execute proper bed making and turn down services.

COMMUNICATION. The student was able to convey message effective through properly communication.

HYGIENE AND SANITATION. The student was able to practice proper hygiene and sanitation procedures.
CUSTOMER SERVICE. The student is able to deliver proper customer relations and is able to handle complains
effectively.
