Description of Incident The incident happened when Mr. Mervin C. Agena, Driver of HOOD - EPC Corporation was driving using
Boom Truck with the of Plate No. WKQ 521 going to BIPH
Unit (LGU Bendia, Pasay) called his attention and accused of smoke belching. Interrogation happens and
Mr. Perolina paid the violation for the amount of Two Thousand Five Hundred Pesos (P2500.00).

What Caused the Incident No one oversees the maintenance of all the vehicles. Management must assign a responsible
person to do the job.

Corrective Actions Taken advise the said vehicle must be check by MOTECH (autoshop) for maintenance purposes.
