In accordance with HSE Plan and risk mitigation for any emergency situation as follow:

• Gas leaking
• Big fire
• Explosion
• Tsunami
• Earth quick or disaster
• Demonstration
EMERGENCY CASE

The purpose of this emergency lay out and Response is to provide a framework for response structure,
organization, responsibilities, planning and management in case of required MEDIVAC during JO RDMP
Construction activities on site. This applies to site area JO RDMP and all its employees. All Contractors
shall follow the JO RDMP ERP within the yard boundaries up to the relinquishing point, specified in the
requirement for the subcontractors, being the appointed hospital, which is Pertamina Hospital. This ERP
is a “live document” and will require periodic review to ensure that it is current and up to date
throughout the project.

For emergency communication lines using radio in channel 1.

The following initial response measures:

• Reporting Personnel’s Identity

• Time and location of event
• Nature of Emergency (e.g. Explosion, Fire, Personnel Injury, Chemical/Oil Spill, etc)
• Injured Personnel Condition (If Any)
• Assistance Required
• Category of event/incident

Alert System in case emergency identified

For emergency case on site or at any other location under JO RDMP Company’s control should be
reported immediately (by priority order) Through general radio call as assigned How to alert and info via
emergency channel to activated the ERT team and mustering as well

The following information should be given:

Always:

Where - Location of emergency

Involved People/Team – name of personnel/team involved in that area

What - Type of incident, situation

How many - Number of injured/sick persons

Victim(s) status - Presence of bleeding, consciousness, breathing, pulse

First aid - Action undertaken by First Aid medical team

Risks - Information about remaining risks at accident scene

Where to go - Nearest mustering, access point to reach by ambulance

Contacts - Phone number and name of the person reporting.
