As required in the CONTRACT OF SERVICES and TERMS OF REFERENCE (TOR), the undersigned SECOND
PARTY for the month of October 2021, has:

1) Provided advice to the Sangguniang Panlalawigan on the relevant provisions of the Local Government
Code on the creation of department head positions;

2) Provided pertinent information on the various prohibitions during the election and campaign periods
as provided for in the Omnibus Election Code;

4) Attended weekly meetings of the majority members of the Sangguniang Panlalawigan;

5) Performed other duties and functions which were requested by the SP in the discharge of their
functions.
