SPORTFIT INC., a corporation duly organized and existing under Philippine laws, with
office address at Suite 102 Upper G/F Security Bank, Bldg # 228 Ortigas Ave, Greenhills, herein
represented by its Authorized Representative, Katherine Robles, Filipino of legal age, do hereby
state that:

1. SPORTFIT INC., is a subcontractor for the supply and installation of Pavigym Flooring, for
the project of Sunly Properties, Inc -- the MMC Women’s Wellness Center (the Project) located in
7th Floor, Ayala North Exchange Building, Ayala Avenue corner Amorsolo St., Makati City, Metro
Manila. The said project is owned by Medical Doctors’, Inc.

2. SPORTFIT INC., guarantees the quality of materials and workmanship (labor) used and
executed in the Project within one (1) year starting from March 13, 2020 - the date of its acceptance
by the Owner, Medical Doctors’ Inc..

3. SPORTFIT INC., shall assume liability for faulty and/or defective materials or
workmanship, effective for a period of one (1) year starting from the date of acceptance by the
Owner. The remedy of the faults on its part or defects in materials or workmanship, if any, due to
contractor’s own doing shall be undertaken within a minimum of five (5) working days depending
on the complexity of the rectification work.
