AFFIDAVIT OF CONSENT

I, JOANNA MARIE DE GUZMAN NARIDO, of legal age, married to

JASON CANLAS NARIDO and a resident of Purok 3, Brgy. Bonifacio,
Cabanatuan City, Nueva Ecija after having been duly sworn to in accordance
with law hereby depose and states the following:

1. That I have two (2) children namely DANIELLA NICOLE DE

GUZMAN who was born on August 08, 2012 and

____________________;

2. That in case I am voluntarily and without force, threat or

intimidation consent to the adoption of my daughter by my
husband, JASON CANLAS NARIDO;

3. That the only consideration for the adoption of the said adoption
of my daughter is to give her a bright future and a comfortable life;

4. That I signify my written consent, freely, willingly and

voluntarily to have my daughter DANIELLA NICOLE DE
GUZMAN be legally adopted by my husband, JASON CANLAS
NARIDO in accordance with law;

5. That I am executing this affidavit to declare my full consent to the

adoption of my daughter DANIELLA NICOLE DE GUZMAN as
well as to the truth of the foregoing facts.
