AFFIDAVIT

been duly sworn to in accordance with law do hereby depose and state:

1. That I am the Owner OR Treasurer of _____________________,

CORPORATION which is an entity duly established under the
laws of the Philippines;

2. That our company/business is involved in providing driving lessons

for prospective clients which is a requirement in securing a driver’s
license;

3. That I hereby certify that we have adequate funds to operate this

driving school at least for the nect three (3) years as evidenced by
the following documents:

a. Certification from ___________________ bank to prove that

we have a deposit of _________________ million pesos;

b. Copy of Passbook pertaining and indicating the above

mentioned deposit

4. That in addition, we commit, under pain of sanctions provided for

by law that the deposit will be utilized solely for the operations of
the driving school which we will operate;

5. That I am executing this affidavit to attest to the truth of the

foregoing and for whatever legal purposes it may serve.
