AFFIDAVIT OF DISCREPANCY

I, GLORIA ABERGAS BALAJADIA, of legal age, Filipino and a resident

of Brgy. Rizal, Santa Rosa, Nueva Ecija, after having been duly sworn to in
accordance with law hereby depose and states:

1. That in a Deed of Donation which the affiant executed sometime

last November 2017, it was inadvertently written that my name is
GLORIA ABERGAS;

2. That in truth and in fact, my true and correct name is GLORIA

ABERGAS BALAJADIA and GLORIA ABERGAS is my maiden
name;

3. That GLORIA ABERGAS BALAJADI

5. A and GLORIA ABERGAS refer to one and the same person and to
prove the true full name of the affiant, attached herewith is a
photocopy of his Senior Citizen Identification Card as ANNEX
“A”;

6. That I am executing this affidavit to attest to the truth of the

foregoing fact and for whatever legal purpose this may serve.
