RE: NOTICE OF FINAL DEMAND

Dear Mary Cris E. Corminal,

The undersigned writes in behalf of my client, Christ The Healer Hospital,

of Sogod, Southern Leyte, regarding your ungrateful and irresponsible acts towards
my client. As narrated by my client, you are admitted and treated for four (4) days
and after all, you run away from the hospital premises without paying your hospital
bills.

As of this date, you incurred an amount of Ten Thousand Two Hundred

One Pesos and Seventy Centavos, (P10,201.75).

This letter serves as FIRST and FINAL DEMAND for you to pay my client
within FIVE days from receipt thereof. OTHERWISE, we will be constraint to
seek Court/Judicial remedies to protect the interest of my client. It is understood
that at the moment you run away your obligations for the Christ the Healer
Hospital, DAMAGES arises and YOU ARE LEGALLY LIABLE on that.

Please give this matter your due attention and preferential action before its
too late.
