LETTER TO THE PRECEPTOR

Dear Mrs. Minu Mathew,

My name is Merin Anna Mathew, and I am a first-year student in the OTA/PTA (Occupational
Therapy Assistant and Physiotherapy Assistant) Program at Durham College. Lisa Kowal, our
placement officer, gave me your contact information and informed me that from January 17th to
March 30th, 2020, I will be doing my level I placement with you at MSM Physiotherapy and
Rehabilitation Centre in Oshawa on Mondays and Wednesdays.

I'm writing to express my gratitude for the chance to work with you on my future field placement.
I am extremely excited about this placement and am highly interested in the fields of physiotherapy
and rehabilitation, and I assure you that I will do my best to remain honest and responsible within
the scope of practise. I'd also like to know if there's anything else you suggest I look at or do before
the first day to help me prepare for this placement.

I wanted to confirm the dress code and whether scrubs or business casual are appropriate, as well
as to confirm that we will be meeting on January 17, 2021, at 9 a.m. at MSM Physiotherapy and
Rehabilitation Centre in Oshawa.
