Should include :
- brief description of their chosen family and the rationale for family of choice
- reference or citation.

Chapter II - Family Profile, environment and family health behavior

This contains the detailed and narrative description of the family health
assessment form that includes 5 major clusters of data .
I. Family Characteristics, mobility and dynamics
II. Socioeconomic and cultural characteristics
- Social Integration, cultural influences, values, attitudes and beliefs
III- Family environment - residence and neighborhood
IV- Family health and health behavior – ADLs, Health history, Self Care, Risk
Behavior, Health Status, Home remedies and health care resources.
