REGISTRATION FORM

The registration form first opened and accepted responses on November 6, 2021 and
stopped accepting responses on November 13 at exactly 11:59 AM.

The total number of people who registered on the form was 853 responses.

652 or 76.4% of them were students while 188 or 22% were guests and 13 or 1.5% were our
faculty/clinical instructors.

682 of them were male or 80% and 171 or 20% were female. 501 of them gave their contact
numbers to us which is optional.

In the PLMar year and sections there were 51 (7.9%) student in 1BSN1, 66 (10.2%) students in
1BSN2, 46 (7.1%) students in 1BSN3, 51 (7.9%) student in 1BSN4, 34 (5.2%) students in
1BSN5, 29 (4.5%) students in 1BSN6, 40 (6.2%) students in 1BSN7, 25 (3.9%) students in
1BSN8, 34 (5.2%) students in 1BSN9, 37 (5.7%) student in 1BSN10, 31 (4.8%) students in
2BSN1, 35 (5.4%) students in 2BSN2, 35 (5.4%) students in 3BSN1, 28 (4.3%) students in
3BSN2, 62 (9.6%) students in 4BSN1 and a 45 (6.9%) students in 4BSN2 with a total of 649
responses.
