The MCT and MST will use this form to formally observe the trainee’s performance and to give feedback
based on the selected teaching competencies.
NOTE: Refer to the course-specific, teaching competencies-based rubric included in section D of the TP Booklet
Personal goal should be something related your own teaching skills, not the lesson goals. You
added too many personal goals. Please make two personal goals.
Add the words you will cover in the lesson (Student will be able to say words that start with the
letter I.) Lesson outcomes need to be more specific. Remember, they need to be SMART
(measurable).
Add the questions you will ask your students in each stage in the lesson plan.
