Less Urinating
A 50 years old man presented to the clinic because of his urination problem. Here, in the next paragraph
contain of the conversation between patient and doctor in charge.
Doctor : Good morning, what can I do for you?
Patient : Good morning doctor. I have a problem of decreased urinating during 3 month. The urine
volume around 2 cups a day only.
Doctor : Besides decreasing volume of urination, can you describe your urine, such as: color,
clarity, foamy etc.?
Patient : The urine is rather reddish, somewhat turbid, and foamy.
Doctor : Do you have any history of Illess?
Patient : Yes, I have, I suffered hypertension and diabetes since last 5 years, and I took routine
medicatons.
Doctor : Now, I want to perform physical examniations, and after that I will send your urine and
blood to the laboratory. Are you agree?
Patient : Yes, doctor, I agree.
Doctor was doing the physical examination and sending specimens of uriene and blood to the laboratory
Doctor : I will show you the result of physical and laboratory examination.
Physical examination showed blood pressure of 170/100 mmHg, heart rate of 84 x/min, and
respiration rate 24 x/min.
Urine examination showed: 3+ of erythrocyte, 3+ of protein, positive granular cast and 2+ of
glucose.
Blood examination showed: blood urea 50 mg/dL, creatinine 6 mg/dL and potassium 5 mmol/L.

Contoh penyakit akibat Urinary Tract Infection
a. Cystitis
Bladder inflammation. Clinical syndrome of dysuria, urinary frequency and urgency, which
may be accompanied by supra-pubic tenderness. The commonest cause of the inflammation
is infection, but not exclusively. These symptoms and signs can be termed those of the ‘lower
urinary tract’, and the infection has not ascended beyond the bladder.
b. Urethritis
Inflammation of urethra, usually due to infection. Associated symptoms may mimic those of
cystitis. Infective causes, such as Neisseria gonorrhoeae or Chlamydia trachomatis, are usually
acquired during sexual contact and do not ascend to the bladder, so although the urethra is
normally the final passageway for urine from the body (and the primary route for organisms
to gain access to the bladder and beyond), specific urethral infections are not usually included
in the term UTI.
Rute infeksi
a. Ascending infection
The most common route of infection is ascension of bacteria from the urethra to the bladder.
Early animal studies supported this, showing that if bacteria were directly instilled into the
bladder, and one ureter was ligated, the unligated kidney was more likely to develop
pyelonephritis.6 The most common bacterial causes of UTI are the same bacteria that colonize
the gut and they enter the urinary tract following colonization of the peri-urethral area.7 Once
within the bladder, bacteria may then multiply and ascend the ureters to cause upper UTI.
b. Haematogenous infection
Less commonly, upper UTI can be a result of haematogenous spread of bacteria, for example
in prolonged bacteraemia, often associated with a deep source of infection such as
endocarditis. Direct animal studies support this, showing that intravenous injection of
Staphylococcus aureus can result in pyelonephritis.8 However, it is more difficult to produce
pyelonephritis in similar models with Gram-negative bacteria, suggesting that this is not the
common route of infection for most pathogens.

Flank pain (may radiate Costovertebral angle Sudden renal edema, resulting in

constant) distension.
Treatment
Treatment with antimicrobials aims to eradicate the bacteria causing infection. The chosen
antimicrobials depend on extent of infection (uncomplicated or complicated), common local
pathogens, and resistance patterns. Examples of antibiotics for uncomplicated UTI include:
a. Trimethoprim-sulfamethoxazole
Inhibition of microbial DNA synthesis by inhibiting the folic acid synthesis and consequently
the purines required for DNA
b. Fluoroquinolones
Inhibition of microbial DNA synthesis by blocking DNA gyrase and topoisomerase IV needed
for successful DNA replication and transcription.
c. Nitrofurantoin
The mechanism is not fully understood, but it directly causes selective damage to microbial
DNA, which metabolises the toxic intermediates of nitrofurantoin more rapidly than human
cells.

a. Pyelonephritis akut
Penyebab
E. coli, Klebsiella, Proteus, Enterobacter. Sebagian besar pasien menerima organisme
infeksius dari flora fecal.
Rute
Hematogenous – kurang umum, biasanya terjadi dengan obstruksi ureteral (bisa
terjadi karena sesuatu seperti batu ginjal ) atau pasien yang tertekan imunnya atau
debilitated
Ascending infection – bakteri menempel pada sel epithelial mukosa urethral ->
berjalan ke bladder via urethra
Klasifikasi
Tidak komplikasi (uncomplicated)
Komplikasi (complicated) – pasien hamil, pasien dengan diabetes yang tidak
terkontrol, transplantasi ginjal, abnormalitas anatomi urinary, gagal ginjal kronis dan
akut, pasien yang tertekan imunnya dan infeksi bakteri yang didapat di RS.
Bisa diakibatkan oleh :
- Obstruksi tractus urinary – sesuatu seperti batu ginjal. Obstruksi aliran keluar
urin bisa mengarah ke pengosongan yang tidak selesai/komplit dan stasis
urinary – bakteri bertambah banyak tanpa dikeluarkan
- Vesicoureteral reflux – kondisi kongenital dimana urin mengalir kembali dari
bldder ke ginjal
Komplikasi
Formasi abses perinephric atau renal, sepsis, thrombosis vena renal, nekrosis papilari, gaga;
ginjal akut, emphysematous pyelonephritis (EPN)
E. coli memiliki kemampuan untuk menempel dan mengkolonisasi tractus urinary dan ginjal
dengan molekul adhesive bernama P-fimbriae, yang bisa berinteraksi dengan reseptor di
permukaan sel uroepithelial. Selanjutnya ginjal mengalami inflamasi akut dan menyebabkan
scarring parenkim ginjal. Dianggap penempelan bakteri ke sel ginjal mengganggu penghalang
protektif yang mengarah ke infeksi lokal, hipoksia, iskemi, dan penggumpalan. Selanjutnya,
sitokin inflamasi, toksin bakteri, dan proses reaktif lain bisa melengkapi pyelonephritis dan
terjadi juga gejala sistemik (sepsis dan shock)
b. Pyelonephritis kronis
Hasil dari pyelonephritis akut yang komplikasi/konsekuensi dari pyelonephritis akut berulang/
yang tidak ditangani
Etiologi yang mendasari
Abnormalitas kongenital dari ginjal dan tractus urinary (terlihat pada anak-anak, bisa
unilateral atau bilateral)
Obstruksi yag didapat seperti yang terlihat pada adanya batu, hipertrofi prostatm
limfadenopati, fibrosis retroperitoneal, atau neurogenic bladder
Pyelonephritis kronis karena adanya abnormalitas structural terjadi lebih sering pada infant
dan anak berusia <2 tahun
5-6% anak anak dengan pyelonephritis kronis karena vesicoireteral reflux berkembang
menjadi kompllikasi jangka panjgan seperti hipertensi dan bisa berkembang menjadi gagal
ginjal
Tak hanya itu, pyelonephritis kronis bisa menyebabkan end-stage renal disease yang terjadi
pada anak-anak
Pada orang dewasa: pyelonephritis kronis bisa mengarah dari diabetes, immunocompromise,
nephrolitihiasis, obstruksi, reflux, atau neurogenic bladder
Ketika terjadi pyelonephritis kronis terjadi secara bilateral, penyakit ginjal kronis bisa terjadi
Komplikasi
Proteinuria, glomerulosclerosis fokal, scar renal progresif yang mengarah ke ESRD,
Xanthogranulomatous pyelonephritis (XPN) yang sulit dibedakan dengan kanker ginjal,
pyonephrosis (bisa terjadi pada kasus obstruksi), scar ginjal progresif (reflux nephropathy)
e. Glomerulonephritis
Glomerulonephritis merupakan penyakit ginjal spesifik hasil dari mekanisme imunologi yang
memicu terjadinya inflamasi dan proliferasi jaringan glomerular sehingga terjadi kerusakan
pada membran basal, mesangium, dan endothel kapiler. Klasifikasi etiologi dibagi menjadi 2
berdasarkan manifestasi klinis yang terlihat, yaitu:
Nephrotic glomerulonephritis, dengan proteinuria berat (>3,5 g/hari) dan edema
Etiologinya dapat berupa minimal change disease, focal segmental
glomerulosclerosis, membranoproliferative glomerulonephritis, membranous
nephropathy, HIV associated nephropathy, diabetic nephropathy, dan amyloidosis
Nephritic glomerulonephritis, dengan hematuria dan hypertensi, serta proteinuria
yang kurang terlihat.
Etiologi
IgA nephropathy, Henoch Schonlein Purpura (HSP), post streptococcal glomerulonephritis,
anti-glomerular basement membrane disease, lupus nephritis, hepatitis C infection, dll.
Patofisiologi
Meskipun etiologi dari jenis glomerulonephritis beragam, mekanisme patogenik umum
dimediasi oleh kerja sistem imun, baik sistem imun humoral ataupun aktivasi cell-mediated
pathway. Akibatnya respon inflamasi yang terjadi dalam banyak kasus akan diikuti dengan
kejadian fibrotik (Kazi & Hashmi 2020). Lesi pada glomerulonephritis terjadi akibat deposisi
glomerular ataupun fomrasi in situ dari kompleks imun. Secara makroskopis ginjal akan
mengalami pembesaran hingga 50%, sedangkan secara histopathologis akan terlihat adanya
pembengkakan berkas glomerulus dan infiltrasi polimorfonuclear serta adanya deposisi dari
immunoglobulin dan komponen komplemen (Parmar 2018).
Pada Post Streptococcal Glomerulonephritis (PSGN), dilaporkan adanya potein turunan dari
Streptococcal. Neuramidase dari Streptococcal akan mengubah immunoglobulin G (IgG) dari
host, kemudian IgG akan bergabung dengan antibodi dari inang. Kompleks imun IgG/anti-IgG
akan terbentuk kemudian terakumulasi pada glomerulus. Selain itu, peningkatan titer
antibodi yang lain seperti Anti Streptolysin O (ASO) atau Anti Hyaluronidase, DNAase-B, dan
Streptokinase, juga dapat menjadi bukti adanya infeksi Streptococcus baru-baru ini (Parmar
2018).
Sedangkan pada glomerulonephritis yang terkait dengan infeksi Staphylococcus, deposit
sistem imun yang terlibat dominan atau co-cominant adalah IgA dan komponen komplemen
C3 (Parmar 2018).
Pada PGSN dan reaksi hipersensitivitas III lain, sel endothel vascular pada ginjal akan secara
aktif mengangkut kompleks imun yang bersirkulasi dari kapiler menuju ke spatium interstitial
peritubular dimana nantinya akan dideteksi dan diambil oleh resident macrophag.
Pengambilan kompleks imun oleh resident macrophag ini nantinya akan memicu pelepasan
sitokin pro inflamasi, dimana akan memicu proses prekrutan monosit dan neutrophil dari
sirkulasi ke dalam ginjal (Parmar 2018).
Komplikasi
Komplikasi glomerulonephritis nantinya dapat mengarah menjadi Acute Kidney Injury (AKI)
atau dapat berkembang secara bertahap menjadi Chronic Renal Failure. Kadang, tampakan
awal terjadinya AKI berupa glomerulonephritis yang berprogresi cepat. Selain itu, vasculitis
dan Goodpasture syndrome juga mewakili kondisi dimana AKI berhubungan dengan
glomerulonephritis. Pada sebagian besar kasus, kondisi ini menunjukkan perkembangan
menjadi CKD serta perlu dilakukan terapi dialysis (Kazi & Hashmi 2020).
Komplikasi lain yang mungkin terjadi adalah peningkatan tekanan darah akibat kerusakan
pada ginjal yang menyebabkan penumpukan sampah-sampah metabolisme dalam aliran
darah. Selain itu dapat dijumpai nephrotic syndrome dengan protein berlebih yang dijumpai
pada urin, sehingga protein dalam darah menjadi sangat sedikit. Nephrotic syndrome
berkaitan dengan tingginya level kolesterol dalam darah serta pembengkakan/ edema pada
kelopak mata, kaki, dan abdomen (Mayo Clinic Staff 2020).
f. Renal failure
Patofisiologi
Untuk yang gejala akut dapat diklasifikasikan ke 3 mekanisme
Berkurangnya blood flow renal (prerenal azotemia):umumnya muncul pada reduksi
absolut dari volume cairan ekstraseluler atau reduksi dari circulating volume walau
total volume cairan normal.Umumnya ginjal memiliki mekanisme auto regulasi untuk
menjaga tekanan intra kapiler saat fase awal yang akan menyebabkan dilatasi arteri
afferent dan konstriksi arteri efferent.Saat kondisi prerenal ini menajdi makin
serius,mekanisme ini pun akan gagal untuk mengkompensasi penurunan GFR dan
kenaikan level BUN dan creatinine
Intrinsic renal parenchymal disease (renal azotemia): kelainan intrinsic dapat dibagi
tergantung bagian mana yang terlibat seperti glomeruli, vasculature, atau
tubulointestinum
Obstruksi dari renal outflow (postrenal azotemia)
Untuk gejala kronik akan melibatkan mekanisme kompensasi yang berujung pada hiperfiltrasi
dan hipertrodi dari nephron yang tersisa. Saat hal ini terus berlanjut, akan terjadi perubahan
histopatologis dan akan menyebabkan gangguang dari struktur glomerulus, abnormalitas
fungsi podosit, dan gangguan dari filtrasi yang berujung pada sklerosis
Komplikasi

Hyponatremia
Hyperkalemia
Acidosis
Calcium and phosphate balance
Anemia

g. Oliguria
Kondisi dimana berkurangnya jumlah urin yang dialami oleh pasien disebut dengan oliguria.
Dapat disebabkan oleh factor prerenal, renal/ intrinsic, dan postrenal
Faktor pre renal:
Hipovolemia: Berkurangnya volume darah karena kurangnya konsumsi cairan,
trauma, surgery, luka bakar, dsb
Pump failure: bisa karena infark myokard, pulmonary embolism, cardiac tamponade,
dan gagal jantung kongestif
Vasculer: oclusi vena atau arteri renalis karena stenosis parah, thrombosis,
thromboemboli, gangguan autoregulasi renal karena ACE inhibitor
Faktor renal/intrinsic:
Glomerulonephritis, vasculitis, interstitial nephritis, scleroderma, hipertensi maligna
Nekrosis tubular akut karena iskemi atau substansi nephrotoxic
Faktor post renal:
Obstruksi upper urinary tract

Pemeriksaan Untuk Mengevaluasi Fungsi Renal → Renal Function Test (Gounden, Bhatt, Jialal,
2020)
a. Glomerular Filtration Rate (GFR)
Merupakan indikator terbaik untuk cek fungsi glomerulus, ngecek clearance suatu substansi
dari darah
GFR normal: 90-120ml/menit (laki2 dewasa). Kalau <15 ml/menit mengindikasikan gagal
ginjal tahap akhir dan butuh renal replacement therapy (mis. dialisis)
Karakteristik marker yang ideal untuk memeriksa GFR:
Harus muncul secara endogen di plasma pada laju yang konstan
Harus terfiltrasi secara bebas di glomerulus
Tidak bisa direabsorpsi atau disekresi tubulus ginjal
Tidak melalui eliminasi ekstrarenal
Endogen marker gada yg gitu, adanya eksogen → inulin, radioisotop (51 Cr-EDTA, 99 Tc-
DTPA), iohexol (paling menjanjikan, kontras non radioaktif, aman untuk anak2). Tapi pake
marker eksogen itu inconvenience, jadinya kita pakai marker endogen yang mendekati
kriteria tersebut
Marker endogen:
Kreatinin
- Produk samping kreatin fosfat di otot, diproduksi scr konstan oleh tubuh,
dibersihkan sepenuhnya dari darah oleh ginjal. Jumlahnya tergantung massa
otot, diet, kehamilan (lebih rendah).
- Indikator kalau terdapat gangguan ginjal dimana fungsi ginjal menurun hingga
50%.
- Rumus: C = (UxV)/P
- C : clearance, U : urinary concentration, V : urinary flow rate (ml/min), P: plasma
concentration
- Harus dikoreksi untuk area permukaan tubuh
- Overestimate GFR 10-20%
Blood Urea Nitrogen (BUN)
- 85% urea dieliminasi lewat ginjal, sisanya lewat GIT
- Urea serum meningkat ketika clearancenya menurun pada gagal/gangguan
ginjal akut dan kronis, perdarahan GI atas, dehidrasi, status katabolik, diet tinggi
protein
- Menurun: kelaparan, diet rendah protein, penyakit hati parah
- Kreatinin lebih akurat, tapi urea meningkat lebih awal di penyakit ginjal
- Rasio BUN:kreatinin → prerenal = 20:1; intrinsik = 10:1
Cystatin C
- Dibentuk dalam laju konstan, bebas difiltrasi ginjal
- Korelasi dengan GFR berkebalikan. GFR rendah, cystatin C tinggi
- Setelah difiltrasi akan direabsorpsi dan dimetabolisme oleh tubulus proksimal,
jadi di kondisi normal, cystatin C ga masuk ke urin final yg diekskresikan
- Keuntungan dibandingin kreatinin: tidak dipengaruhi usia, massa tubuh, diet
b. Albuminuria dan Proteinuri
Albuminuria → keberadaan abnormal albumin di urin, penanda deteksi nephropathy yang
baru terjadi pada pasien diabetik, penyakit cardiovaskular, dan penanda kerusakan ginjal
kronis.
Spesimen: urin 24 jam atau morning-specimen atau random specimen
Ada albumin → indikasi disfungsi glomerulus
Klasifikasi KDIGO albuminuria:
A1 : <30 mg/g kreatinin
A2 : 30-300 mg/ g kreatinin
A3 : >300 mg/g kreatinin
Proteinuria : >300mg/hari (normalnya 150mg/hari: 30% albumin, 30% globulin, 40% Tamm
Horsfall protein)

Pengukuran osmolalitas urin untuk mengetahui kemampuan mengkonsentrasikan urine oleh
tubulus
Osmolalitas urin >750 mOsmol/Kg H2O : NORMAL
Tes ammonium klorida juga bisa untuk konfirmasi diagnosis asidosis tubulus distral ginjal
dengan kegagalan mengasamkan urin untuk mencapai pH <5.3.
d. Analisis urin
Terdiri dari observasi fisik/makroskopik, kimiawi, dan pemeriksaan mikroskopis urin
Urinalysis: eritrosit (3+), protein (3+), granular cast (+), glukosa (+)
Interpretasi:
Protein 3+ → 300mg/dL → tinggi (proteinuria)
Granular cast → tanda berbagai penyakit ginjal
Eritrosit + → tanda ada renal injury
Reference range (Lerma, 2015):
Color – Yellow (light/pale to dark/deep amber)
Clarity/turbidity – Clear or cloudy
pH – 4.5-8
Specific gravity – 1.005-1.025
Glucose - ≤130 mg/d
Ketones – None
Nitrites – Negative
Leukocyte esterase – Negative
Bilirubin – Negative
Urobilirubin – Small amount (0.5-1 mg/dL)
Blood - ≤3 RBCs
Protein - ≤150 mg/d
RBCs - ≤2 RBCs/hpf
WBCs - ≤2-5 WBCs/hpf
Squamous epithelial cells - ≤15-20 squamous epithelial cells/hpf
Casts – 0-5 hyaline casts/lpf
Crystals – Occasionally
Bacteria – None
Yeast - None
Pemeriksaan makroskopik:
Lihat warna, kejernihan
Warna normal: kuning pucat; dehidrasi → lebih gelap; merah → hematuria atau
porphyria; cloudy → pyuria karena UTI
Pemeriksaan kimiawi:
Massa jenis normal : 1.003-1.030, diukur dengan refractory metri atau dipstick
Dipstick juga bisa ngecek protein, glukosa, darah, keton, bilirubin, urobilinogen, nitrit,
dan leukosit esterase
Glukosa ga kedeteksi di pasien sehat, ada di pasien diabetes mellitus, hamil, dan renal
glucosuria
Darah bisa ada setelah renal tract injury/infection
Keton ada waktu puasa, muntah parah, diabetic ketoacidosis
Bilirubin di urin normal harusnya ga ada, ada kalau kedeteksi conjugated
hyperbilirubinemia
Nitrit dan leukosit esterase indikator UTI
Protein: 1+ (30 mg/dL); 2+ (100 mg/dL), 3+ (300mg/dL), 4+ (>1000 mg/dL)
Pemeriksaan mikroskopis
Deteksi sel, cast, kristal, mikroorganisme
Red blood cast → glomerulonephritis; white blood cast → pyelonephritis; hyaline
cast → terdiri dari protein, bisa terjadi di glomerular disease; fatty cast → sindrom
nephrotic
WBC: 0-5 sel/HPF; RBV: 0-3 sel/HPF
Ada RBC → renal injury; WBC → infeksi
Kristal:
Triple phosphate crystal : ‘coffin-lid’ appearance, terlihat pada urin alkalin dan UTI
Kristal asam urat : seperti jarum, diasosiasikan dengan gout
Kristal oksalat : seperti amplop, ada di ethylene glycol poisoning atay hiperoxaluria
primer dan sekunder
Kristal cystine : heksagonal, ada di cystinuria
Spesimen sebaiknya yg freshly voided midstream urine, yang tidak terkontaminasi bakteri
komensal dan sel epitel

Berdasarkan skenario, dimana pasien memiliki kadar kreatinin serum = 6 mg/dL dan umur = 50
tahun, maka berdasarkan rumus MDRD eGFR nya yaitu:
eGFR = 186 x (SCr)–1,154 x (Usia)–0,203
eGFR = 186 x 6–1,154 x 50–0,203
eGFR = 11 ml/min/1.73 m2
Karena nilai eGFR pasien kurang dari 15 ml/min/1.73 m2, maka pasien sudah termasuk mengalami
renal failure.
