occupation – medical receptionist.

married – has 2 children.
lives with husband.
occasional drinks alcohol.
non-smoker.
no known allergies.

Family history:

father – unknown history.

mother – migraine.
grandmother – died of stroke.

10/01/1997:

headache,30 min – vomiting 4-5 times/day

2-day history of aura 3-5 min preceded by photophobia – phonophobia
OTC Ibuprofen

Assessment:

migraine suspected.
vicodin 20 mg

01/02/1998:

migraine attack – varieties of symptoms

Ibuprofen – vicodin 50 mg

15/10/2010: 

Subjective: migraine improved – no more attacks – low mood

Objective: unremarkable

Assessment: depression

Treatment:

Zoloft 20 mg
continue Ibuprofen – vicodin 50 mg

01/11/2010:

worsen depression
Zoloft 50 mg

11/07/2020:

Subjective:

decreased vision over computer o ce, blurred vision for 3 days.

severe bifrontal headache 5/7, not like migraine.
unable to move RT eye, painful movement of both eyes more at RT eye
double vision in both eyes
her husband noticed bulging of right eye
upper respiratory tract infection week ago
No fever, neck stiffness, SOB, nausea, vomiting, urinary, joint pain, ear symptoms

Objective:

BP 127/83, Temp 37.6, RR 16, Pulse 80, BMI 31

RT eye ptosis – no eye protrusion
chest: normal
heart: normal
Neurological examination : uent, clear speech,
sensation, power, re exes NAD
no signs of eye infection

Plan: refer to neurologist for urgent neurological assessment
