An Alexis of Tau Gamma Phi, is hereby with my own free, good physical and emotional
condition to submit myself to the disciplinary initiation of the Tau Gamma Phi/Triskelion
Grand Fraternity. I have also read the Anti-hazing Law (Republic Act No. 8049) stating
that “AN ACTREGULATING HAZING AND OTHER FORMS OF INITIATION RITES
IN ANY FRATERNITIES, SORORITIES, AND OTHER ORGANIZATIONS AND
PROVIDING PENALTIES THEREFORE.” and fully understood about the law written
under its article.

And that, on doing so, I shall blame no one except myself for whatever happens to
me on this day and the days to come, for wishing to achieve the honor of being a member
of the Tau Gamma Phi/Triskelion Grand Fraternity.
