PARENT CONSENT FORM-AUGUST 2021

Dear Parents,

As per the latest circular given by State government, schools have to take
written consent from parents regarding reopening and conduction of offline
classes for classes 9-12th.

Hence to take your opinion we are conducting the survey regarding the
same. Your response will help us to plan and prepare for the safe re-
opening of the school. We would like to keep you informed that we have
taken precautions to ensure that school meets the prescribed health and
safety standards.

Given the unknown nature of COVID-19, it is not possible to fully list each
and every individual risk of contacting COVID-19. Hence the school will
not be responsible in case the child gets infected by it in future.

Parents will have to make arrangements to drop and pick up their ward
from school.

You are requested to take the survey and answer the questions given in the
Google Form, the link for which is shared below:
