MOSQUITOES: LIFE-CYCLE
Mosquitoes undergo complete metamorphosis
(egg →larva→pupa→ adult )

14
WHO, 2010
MOSQUITO LIFE CYCLE
❖ Mosquitoes go through four stages in their life
cycle: egg, larva, pupa, and adult or imago.
❖ The first three stages are aquatic.
❖5–14 days, depending on the species and the
temperature;
❖Adult females lay their eggs in water.
❖Eggs hatch to become larvae, then pupae.
❖ Then, the adult mosquito emerges from the pupa
as it floats at the water surface.
Mosquito Borne Diseases.
• Malaria.
Causative agent : P. falciparum, P.vivax,
P. malariae
Vector : Anopheles species.
• Filariasis.
Causative agent : Wuchereria bancroti, Brugia malayi
Vector : Culex quinquefasciatus and Marsonia species.
National Strategies for Malaria Elimination

• SPECIAL AREA STRATEGIES

– Tribal Areas

– Inaccessible and remote areas

– Migrant/Mobile populations

– Urban and periurban areas

– Refugee camps/populations

– Post natural disaster strategies

National Strategies for Malaria Elimination

• Challenges to Malaria Elimination

– Financial sustainability
– Uninterrupted supply of materials – procurement and supply
uncertainity
– Highly trained and skilled manpower at all levels – hiring and
continuation
– Developmental activities creating more malariogenic potential –
identification and prevention
– Engagement of private sector
– Drug resistance
– Insecticide resistance
– Research for newer drugs, insecticides
– Real Time reporting from remote and inaccessible areas

Lymphatic Filariasis

• elephantiasis
• grossly disfiguring
• profoundly disabling
• with serious social, psychological
and economic consequences
• rarely life threatening
• recognized for centuries
• affecting people of all ages and sexes
• particularly poor and vulnerable
• in both urban and rural areas
• neglected disease
• 2nd leading cause of permanent and
long-term disability next only to Leprosy
Age and Sex wise Prevalence of
Lymphatic Filariasis

• Maximum mf prevalence : 15-35 years

• Chronic Cases : > 40 yrs.
• Youngest age
mf positivity : 9 months
Disease : 3 yrs.
• Male / Female Ratio : 1.4: 1
Types of Infection
1. W.bancrofti (bancroftian filariais)- 99.94%
infections
(40% of the global burden of LF)
2. B.malayi (Malayan filariasis, Brugian
filariasis)- 0.06%, widely distributed along
coastal districts of Kerala*
3. B. timori- Southeast Indonesia
* Some of the Old B.malayi foci disappeared
Bancroftian filariasis
• Wuchereria bancroftia
• Normally resides in the lymphatics
• Female worms produce microfilaria
• Two biologically different forms
– Nocturnal periodicity- 10pm –2am
– Diurnal- Continuosly but peak in day time
Microfilaria of Wuchereria bancrofti
Microfilariae of Brugia malayi
Microfilaria Carriers

• Asymptomatic
• Sub-clinical internal damage to their
lymphatic and renal system
• Proteinurea and haematuria
• Periodic
L3 coming out of Mosquito
Adult Worms (Macrofilaria)

• Thread-like worms
Male worm
• Male: 4-6 cms
• Female: 6-12 cms
• Male-Female Sex
ratio: 1:8
• Location: Lymphatics

Female worm • Life Span: 10-15 years

• Fecundity: 3-4 years
Vectors of LF
• W.bancrofti :
Culex quinquefasciatus :Mainland
(nocturnal periodicity)
Oclarotatus (Finlaya) niveus : Car Nicobar Islands
(Diurnally sub-periodic)
• B.malayi : Coastal area ( Nocturnal periodicity)
Mansonia annulifera
M.uniformis
Transmission dynamics of LF
Prepatent period :
Time lapse between entry of L3 and first
appearance of microfilaria (7-9 months)
Incubation period :
Time lapse between entry of L3 and first
appearance of clinical manifestations (8-16
months)
Longevity of adult worm : upto10 yrs.; life span up
to 40 years has also been reported (WHO,1987-
manual )
Guidelines for improving performance of low performing
Filaria Survey Unit, Filaria control unit and Night Clinics

➢ Send HR status (deputation and vacant post status of

various cadres of all FSU, NFCU and NC) data to JtDHS
office,Pune.
➢ Plan work according to your Targets.
➢ Take help of Corresponding ADHS, DHO, DMO and FO in
case of man- Power shortage.
➢ Send requirements to DMO/ FO of your
respective unit regarding
1.Night blood smear collection(Lancet, Prickers
etc.)
2. Reporting format /Registers
3. Medicines
Guidelines for improving performance of low performing
FSU , NFCU and NC contd….

4.MMDP kit distribution should be timely.

➢Arrange regular training programs for continuous medical

education of staff and officers about Lymphatic Filariasis .

➢Hydrocele operation complete in this month.

➢Improve Night Blood Smear collection in Night clinics .

➢Regular and complete examination of NBS should be done.
