AFFIDAVIT OF EXPLANATION
I, ELSA ALBA LARA, of legal age, Filipino, married, and a resident at Blk 4 Lot 5
Morning Glory St. GreenBreeze Subdivision, Brgy. Langkaan I, Dasmarinas City, Cavite, after
having duly sworn in accordance with the law hereby depose and say:

That I am a bonafide member of Social Security System (SSS) with No. 0111-0851817-6;

That I am the sole and surviving legal child of late ALBERTO ALBA who died
understate on June 07, 2013 in Zamboanga City;

That my father travelled to Zamboanga City just to visit his friend on the year of 2010;
since then, we don’t have any connections and communication at all;

That on the time of his death, I was working abroad without knowing he was already
passed away;

That when I came back here in the Philippines on April 2016, I received a information
that my father died on 2013, and immediately coordinate with people who’s in-charge in
my father’s;

That also, I am the one who is taking care and acting as guardian for my mother, who is
having a sick on 2018;

That due to advertence, that the said delaying the claiming the death benefits of my father
in SOCIAL SECURITY SYSTEM is not intended, fraudulent or any acts of illegal
against the law;

That I am executing this affidavit to attest to the truth of the foregoing facts for all legal
intents and purposes.

IN WITNESS WHEREOF, I have hereunto set my hand this 8th day of April 2019, at Imus
