Agreement & Customer Authorization and Application Introduction

As of 12/1/2013, the CPUC approved new forms to be used for the Standard NEM
interconnection process. The PG&E Standard NEM program has streamlined its
interconnection process and has split its single Application and Interconnection
Agreement into two forms:

2. Application for NEM Interconnection – Form 79-1151B
These forms are available on our website (http://www.pge.com/standardnem/ )
