STANDARD DRUG DILUTIONS USED IN THE OPERATION THEATRE COMPLEX AND CARDIAC SURGICAL INTENSIVE CARE UNIT

Note
 These infusions are meant only for adults
 Applicable to all operation theatres and specialties including the cardiac surgical intensive care unit
 Label all infusions clearly
 Deair and flush all tubing’s prior to fixing onto the syringe pump
 Indicate the date of preparation and concentration on the label
 The name of the person who has prepared the infusion should be written clearly
 Highlighted in bold are the drug name in column 2 and the final concentration per ml after dilution of the drug in column 4

19 Glycopyrrolate Inj Each 1ml ampule of Undiluted Load in a 2cc syringe

20 Hep Flush 1 ml of heparin contains Add to 500ml NS Mix well
5000units; take 1ml and add 9 ml
saline thus each ml contains
500units of heparin; 1ml of 500
 units heparin is used
21 Insulin Infusion 40 units of Actrapid insulin Add 45 ml NS Prepare in a 50cc
makes 1ml;take 5ml insulin to 4 units insulin/ml syringe

22 KCL Cocktail Each 10 ml ampule of KCL Add 21 ml of Prepare in a 50cc

contains 20meq of KCL; Each saline syringe
2ml of magnesium contains 1g;
Each 2.5ml ampule of
atracurium contains 25mg; Each
ml of fentanyl contains 50 mcg
i.e.100mcg
In a 50cc syringe take 1 ampule

contains 50mg; to prepare 7.5mg/ml
Rocuronium take 5ml from 1
vial and 2.5 ml from another vial

31 Sodium Nitroprusside Infusion SNP is available in a freeze dried Add 45 ml saline; Prepare in a 50cc
powder. Reconstitute with 5ml wrap in a silver foil syringe
saline to give 10mg/ml to prevent photo
oxidation

32 Vaopressin Infusion. Each 1ml ampule of vasopressin Add 38 ml saline Prepare in a 50cc
contains 20 units;2ampules of 1unit/ml syringe
vasopressin are required for the
infusion

33 Vecuronium Inj/Norcuron Inj Each 4mg ampule of Dilute to 4cc Load in a 5cc syringe
Vecuronium is available as 1mg/ml
freeze dried powder; add 2ml
saline to get 2mg/ml of
vecuronium
34 Xylocard Infusion Each 50ml vial of Xylocard
contains 21.3 mg of Undiluted Prepare in a 50cc
 lignocaine /ml a total of 21.3 mg/ml syringe
