Case 1
A 21-year-old female presented with a dental abscess requiring surgical debridement and is in
your office today for a pre-operative clearance. A routine ECG showed slurred upstroke of the
QRS complexes and denies any cardiac related symptoms. What is your diagnosis? What is
the treatment plan?
Case 2:

A 34-year-old man presented for a work physical for his new job as a crane operator. He is a
healthy individual, no previous medical history, no current medications, denies smoking or
alcohol intake. He reports being a member of a health club and exercises frequently. No family
history of cardiac disease. BP today is 178/94, Prior visit 1 month ago BP was 168/90. All other
vitals WNL. What is your diagnosis? What is the treatment plan?
 Case 3:

• A 68-year-old male presents with palpitations, lightheadedness, and weakness. These

symptoms appear to worsen with increased alcohol intake. Medical history is significant
for hypertension being treated with hydrochlorothiazide. On physical exam, the patient
appears uncomfortable. There is an irregularly irregular pulse when palpating the radial
artery. On cardiac auscultation, the first heart sound (S1) is of variable intensity. There
is mild bibasilar crackles on pulmonary auscultation and an absence of peripheral
edema. An electrocardiogram is obtained. What is your diagnosis? What is the
treatment plan?
